Job overview

The ICB is looking for an experienced nurse who has specialist knowledge and experience of working with individuals who have a Learning Disability (LD). You will have worked in a specialised or community setting with experience of the Continuing Healthcare process.

This is an exciting new role within the ICB and the post holder will be responsible for developing this area of work and supporting the ICB in having a better understanding of the needs of people who have a LD.

Main duties of the job

The post holder will be the case manager for people who are within the CHC caseload and have a LD, which will include completing assessments for CHC and developing a care package for those found eligible. 

The role will require a high level of communication and interpersonal skills and the ability to work supportively and effectively with individuals and their families ensuring that they are fully involved throughout the process.

As people may be live both locally and across the Country, the successful candidate will be required to assess and review individuals beyond the boundaries of Hertfordshire.

Working closely with partner organisations, the ICB's staff design, plan and commission health services for more than half a million people and monitor the quality and effectiveness of those services.  The ICB currently has vacancies for two Clinical Leads within the Continuing Healthcare (CHC) team.

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum and maximum payment per annum (pro rata where applicable) in line with Agenda for Change.

The positions can be located at Harlow, Hemel Hempstead or Welwyn Garden City.

Qualifications

Essential criteria
  • Registered Nurse (Adult or Mental Health) – with current registration
  • Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area
  • Post-graduate degree in Management Studies or equivalent
  • Experience of working in the NHS/public body

Experience and Knowledge

Essential criteria
  • Experience of identifying and contributing to safeguarding concerns
  • Significant experience of successfully operating in a politically sensitive environment
  • Demonstrated experience of co-ordinating projects in complex and challenging environments
  • Experience of managing risks and reporting
