Specialist Health Visitor Strengthening Families Bexley 0 to 19 - Band 7 

£49,178 - £55,492 Inc of HCAS per annum pro rata

Full time - 37.5 hours per week

The Band 7 Strengthening Families posts are new roles, developed to provide  personalised care for families who need it most.  This is an exciting opportunity for Health Visitors who wish to build their leadership and management skills whilst delivering evidence-based care to families on specialist and targeted care pathways. 

The post holder will provide direct specialist clinical service for children and families with complex and additional needs across Bexley and will work within skill mix teams of Community Nursery Nurses, Health Visitor Development Nurses and B6 Health Visitors.

The main role for the Specialist Strengthening Families Health Visitor is to be the delegated named HV for children requiring targeted and specialist support as well as contributing to the public health agenda. 

This is achieved through conducting thorough, individualised health needs assessments, planning programmes of care tailored to families’ needs, and effective liaison with colleagues within Bromley Healthcare and with partner agencies.

  • Lead the health visiting team to deliver the Healthy Child Programme by:
  • Carrying out ongoing health needs assessments for all children and families within the community, using professional judgment to allocate each family to the correct level of health visiting care, Universal, Targeted and Specialist pathways
  • The role will hold a caseload of targeted and specialist families and lead in a high-impact area.
  • Overseeing the delivery of Universal service key touchpoints by skill mix team members
  • Delivering evidence-based personalised packages of care to families who need it most
  • Using Outcome measures to evaluate the effectiveness of care delivered, planning and implementing revised packages of care, where required, and closing or escalating packages of care as required for each child.
  • Participating and where appropriate, leading on the Early Help process for children, ensuring timely assessment and that coordinated care is delivered to address the needs of the child and family
  • To provide health visiting support and advice within the clinic setting, according to the service model and guidelines
  • To liaise effectively with colleagues within Bromley Healthcare and partner organisations 
  • Provide specialist knowledge, supervision, advice, and leadership to team members

This is an exciting time to join the Triborough Public Health Nursing service during a period of reimagining how we deliver Health Visiting.  Part of the transformation has been developing new roles for example the Community Staff Nurse and Specialist Health Visitor, and extending the role of the Community Nursery Nurse. 

The aim of the changes is to create career pathways, to produce a sustainable and resilient workforce, and crucially to protect and promote the uniqueness of the Health Visitor role.  All staff members have corporate iHV membership and learning and professional development are prioritised within the services.

In addition to the opportunity to develop personally and professionally within the team, the successful candidate will have the chance to use their creative skills in developing and delivering training and education and will be supported through an annual appraisal, personal development planning, and access to ongoing training.
