We are looking for a care manager assistant to work in the urgent response care management team. The successful candidate will be working towards preventing hospital admission, promoting independence and facilitating early discharge. The successful candidate will need to have excellent communication skills, knowledge of the health and social care environment, and outstanding organisational and IT skills. The role will involve visiting service users in their own homes and completing regular reviews of care.

Main duties of the job

The post holder will work alongside the other care manager assistants  to provide an effective duty, and triage system for new referrals coming into the UR CM Team. 

Good keyboard skills required for information recording, report writing and e-mails.  

The post holder will manage  a caseload of service users , to monitor and review care services in the community. Ensuring that the cost of the care is appropriate to the service users needs.  

The post holder will be responsible for completing reviews at the end of the enablement period, to ensure that service users with long-term needs have ongoing support in accordance with The Care Act 2014.

The post holder must be able to easily access different properties and environments including house boats, rural setting and cluttered environments.  

The post holder will need to be able to appropriately and professionally engage and communicate with a wide range of service users from different health, social and ethnic and cultural backgrounds

Detailed job description and main responsibilities

1.          Communication and relationship skills

 

·            The post holder will be required to have excellent communication skills, both written and verbal.  This will enable them to work collaboratively and effectively with service users, their families, carers, and professionals from different agencies.

 

·            The post holder must be able to appropriately and professionally engage and communicate with a wide range of service users from different health, social, ethnic and cultural backgrounds.  The post holder will be able to establish a supportive and effective relationship to ensure that services users are able to actively participate in the review process.

 

·            The post would require sensitivity and empathy in providing and receiving complex, sensitive or contentious information, where persuasive, motivational, negotiating, training. or re-assurance skills are required.

 

 

2.          Knowledge, training and experience

 

 

·             The post holder will need to have some experience and/or understanding of working in a care or social services environment or organisation.

 

·            Have good IT skills that enable the post holder to use electronic computer systems, to access and input access service user data.  The post holder would also need to be competent to access and use email and raise reports.

 

·            Additional training will be provided to meet service and organisational specific requirements and develop knowledge and skills.

 

The post holder will need to have studied to A’level, Higher National Certificate or vocational qualification level 4 and be confident to use various electronic systems.

 

 

3.          Analytical and judgement skills

 

The post holder will be working directly with service users, their families, carers, Care Managers, internal and external agencies in the community. They will need to analyse social situations and factors that impact on service user’s ability to manage independently in the community and then determine a course of action. They will also need to use their judgement skills to accurately identify areas of concern or unmet need.

 

4.          Planning and organisational skills

 

·            The post holder will have effective work load management skills, be able to prioritise work schedules and complete work in a timely manner.

 

·            The post holder should be able to work in a busy and occasionally challenging work environment whilst being able to maintain a professional and effective service.

 

·            The post holder will maintain a high level of work that reflects the aims, values and provision of care required of the organisation as a whole.

 

 

5.          Physical skills

 

·            A full and clean driving licence is required to enable the post holder to work across the Medway area for service user reviews, meetings and training sessions.

 

·            Good keyboard skills required for information recording, report writing and emails.

 

·            The post holder must be able to easily access different properties and environments including house boats, high rise properties, rural settings and cluttered environments.

 

6.          Responsibility for patient / client care

 

·            The post holder will be required to work on their own initiative with clients in the community and as part of a larger team.

 

·            The post holder will have regular contact with service users and their families, providers, health care workers and other team members.

 

7.          Responsibility for policy and service development implementation

 

·            The post holder will be required to provide feedback based on reviews they complete with service users, their families, carers and other professionals. The feedback would contribute to the development of services provision in accordance with MCH organisational values and local and national policies.

 

 

·            The post holder will be able to contribute to service development and improvement by proactively engaging in audits, internal process and activity reviews.

 

8.          Responsibilities for financial and physical resources

 

The post holder will be responsible for ensuring that services are provided appropriately to service users. And those services are provided in accordance with The Care Act 2014 eligibility criteria.

 

The post holder will be responsible for the implementing and monitoring of care packages, ensuring that the cost of the care calls is suitable for the individual’s level of needs.  They will also need to understand and explain to service users, family and carers the Medway Council Financial Assessment process, and any possible charges attached to long-term care.

 

9.          Responsibilities for human resources (HR)

 

·            The post holder will be responsible for supporting new members of staff during the induction period, particularly around the use of Framework-i.  They will also be involved in providing “shadowing” opportunities to promote the work of the team for staff working in MCH and in other agencies and organisations ie. Medway Council.

·             

10.      Responsibilities for information resources

 

·            The post holder will be responsible for safe and appropriate access to service user data and for ensuring confidentiality is maintained when transferring, discussing or using service user information.

 

·            The post holder will ensure service user information is only accessed and transferred to relevant services and data protection is maintained at all times.

 

·            The post holder will be trained to use electronic data and IT systems that support service user information.

 

 

11.      Responsibilities for research and development (R&D)

 

·            The post holder will participate in service audits and reviews to ensure the service is operation to current local, national and organisational standards.

 

12.      Freedom to act

 

·            The post holder will be able to demonstrate autonomy in their role, but also be able to recognise the need for supervision or support when necessary.

 

·            In accordance with MCH organisational policy and procedure the post holder must be able to identify and report practices and incidents that are inappropriate and/or detrimental to the post holder, colleagues, service users, the service or the organisation as a whole.
