An exciting opportunity has arisen for an enthusiastic and innovative person to work in a community cardiac rehabilitation team. We are particularly looking for someone with either community or cadiology experience. 

The role would suit someone with excellent communication skills, who is keen provide education and motivate patients in their rehabilitation. 

Main duties of the job

·       To support the entry of suitable patients into the community cardiology service

·       To provide evidence-based phase I-III cardiac rehabilitation in line with all national and local guidelines.

·       To take an active part in the exercise and secondary prevention components of the cardiac rehabilitation service held at multiple sites in the Medway area.

·       To answer correspondence and telephone calls from cardiology patients, to provide advice, support and follow up.

·       To visit cardiology patients in their homes or in clinic to review their progress, facilitate medication optimisation and support lifestyle changes.

·       To supervise, manage and support the development of junior staff within community cardiology

·       To take part in the education of other professionals in the field of heart failure/arrhythmia and cardiac rehabilitation

·       When required contribute expertise and clinical support to other staff within community cardiology

·       To contribute actively to the monitoring and audit of the community cardiology service.

  •          We will offer you the choice of two pension schemes; the NHS Pension scheme and the scottish widows group pension scheme.

·       MCH encourages all staff to be double COVID vaccinated to protect themselves, their colleagues, their family and their patients.

At this time, MCH cannot fund or support any further nurses who require the OSCE training. Please do not apply if you require this training.

Person specification

Qualifications

Essential criteria
  • Registered General Nurse (Part 1 of the NMC register)
  • Educated to degree level or on degree pathway
Desirable criteria
  • Accredited cardiac care course at degree level (or equivalent)
  • Advanced communication skills course

Experience

Essential criteria
  • Significant experience post registration
  • Evidence of continuing professional development
  • Experience of health promotion
Desirable criteria
  • Community nursing experience
  • Experience of autonomous working

Special knowledge / expertise

Essential criteria
  • Experience of working with Cardiac patients
Desirable criteria
  • Experience of working with patients who have had myocardial infarction or cardiac surgery
