- Are you a registered dietitian already working with people with diabetes, or would like to specialise within this field?
- Do you have excellent interpersonal skills and sound clinical assessment
skills? - Do you have the confidence to work alone, putting the patient at the heart of what you do; and to get involved within a multi-disciplinary team?
- Are you passionate about supporting patients holistically and working as part of team to support those with diabetes?
- Hours available between three and five days per week.

We welcome applications from both experienced diabetes dietitians, as well as those looking to develop into the ever-changing world of diabetes care. The post holder will work very closely with other members of the diabetes specialist team, to provide specialist dietetic assessment, review and care plans to patients in the community with diabetes.
They will manage a caseload of patients- assessing, planning, implementing and evaluating dietetic interventions and modifying as necessary via a plan of care.
The role will also involve offering expert advice to health care professionals throughout Medway working closely with other teams within Medway Community Healthcare and Medway Acute NHS Trust.
The post holder will also contribute to the strategic planning and delivery of the multidisciplinary Community Diabetes Service, in line with the strategic direction of the Service, organisational and national objectives, as well as engaging with and working alongside local engagement projects e.g. support groups and Diabetes champions according to NICE guidelines
To ensure that dietetic aspects of the service are culturally appropriate for people with diabetes and those at high risk of developing diabetes
To help develop and participate in the delivery of structured education courses for those living with both Type 1 and Type 2 diabetes, delivering evidence based clinical care for people with Type 1 and Type 2 diabetes in groups and 1:1 setting

The post holder must have the following:
- Experience of inter-professional working
- Relevant experience of working in a related medical setting
- Experience of undertaking advanced clinical assessments
- BSc (Hons)/Post Graduate Diploma Nutrition and Dietetics
- Membership of the British Dietetic Association
- Current State Registration
- Broad base of clinical experience with a specific interest in Diabetes
- Awareness of professional issues in the NHS and Primary Care
- Articulate with excellent communication and interpersonal skills
- Evidence of professional development
- Ability to access diverse sites in a timely manner.
To act as a resource and educator for patients, carers, families, team members, GP’s and other professionals within the MDT to provide and advise on evidence based practices, including effective health promotional aspects, in order to fulfil requirements of local and national guidelines thus ensuring best possible care for those with diabetes.
To use effective written and verbal communication in order to transfer sensitive patient information to other healthcare professionals, promoting patient choice and following examples set out in the 'Language Matters' document
Able to provide care autonomously but recognising own limitations and when to refer to a senior member of team.
To nutritionally assess patients with diabetes by interpreting biochemical, clinical and dietary data whilst considering the patients social circumstances, prognosis and treatment.
Be able to make appropriate referral to other professional, charities or support groups.
Adhere to infection control recommendations to provide safety for patients and self.
To contribute to the development of the team and service provision.
To comply with organisational, departmental and the organisations policies and procedures.
To collect and submit all data and statistics as required in a timely and accurate manner.
