Reporting to the Patient Safety Improvement Lead, this post forms part of the Patient Safety Team to support the delivery of the Trust’s Quality Strategy and vision to ensure ‘Best of care by the best of people’ through the delivery of safe, effective and high quality services, that ensure the
people who use our services, their families, carers and visitors receive a positive experience.
Under the direction of the Patient Safety Improvement Lead the post holder will be responsible for providing leadership and expertise to support the investigation of incidents and complaints, ensuring the completion of duty of candour, and the identification of areas for improvement/lessons and best practice and the development of improvement plans.

Supporting a culture that improves quality by enabling the delivery of high quality care through the effective identification of lessons/areas for improvement using SEIPS (Systems engineering initiative for patient safety).
Ensuring the timely and effective investigation of incidents and complex complaints, ensuring that regulatory duties toward duty of candour are completed, family concerns are included with terms of reference and supporting the review of the report via an internal expert panel
Support the development and delivery of the patient safety team programme of work/plan in line with local and statutory requirements, including PSIRF (Patient Safety Incident Response Framework).
Support the collation of documentary evidence needed to support any external assessment in relation to patient safety.
Support actions identified and undertaken to decrease the risk of incidents occurring and to improve outcomes in relation to people who use our service, their families or carers, staff and visitor safety.
Contribute to internal and external Trust publications as required to promote robust incident management and reporting.

Qualifications

- Degree/management qualification or equivalent experience. GCSE (or equivalent) English Language and Maths to grade C
- A clinical Qualification/Registration is desirable
- Quality Improvement Qualification is desirable
Knowledge

- Knowledge of the NHS National Investigations Framework and duty of candour regulations
- In-depth and significant knowledge and experience of quality and patient safety
- Knowledge of quality improvement tools and techniques and an understanding of the human factors in patient safety
Experience

- Able to apply confidentiality and information governance
- Demonstrates up to date, evidenced based knowledge of current clinical and professional issues
- Experience of gathering and interpreting patient safety data and implementing measurable improvement plans
- Experience of decision-making and prioritising in acute healthcare setting
