Main duties of the job

The successful postholder will be expected to develop and maintain links with community health providers and other services which facilitate discharges and provide a consistent point of contact within the Trust for these services working within the Integrated Discharge Team.

To assist the Trust is meeting its statutory organisational requirements and responsibilities in relation to discharge.

Establish and improve professional standards of care in the identification and management of patients with complex discharge needs, across all clinical and non-clinical specialities.

Detailed job description and main responsibilities

To oversee and co-ordinate the discharge caseloads ensuring all patients are discharged appropriately and in a timely manner.

The post-holder will be a key member of the senior nursing team, assisting with the maintenance of structures, systems and processes in place so that admission avoidance and discharge activity is effectively coordinated across Medway NHS Foundation Trust, making appropriate use of available resources with required operational and service quality standards achieved.

To develop and maintain links with community health providers and other services which facilitate discharges and provide a consistent point of contact within the Trust for these services

To provide specialist advice and support to the wards and their multi-professional teams in managing effective patient discharge as an integral part of care management across Medway NHS Foundation Trust

 

To ensure best practice in planning safe and timely patient discharges in order to achieve best outcomes for patients and to optimise hospital bed utilisation and reduce length of stay

 

To develop effective links and relationships with relevant professional partners, to ensure that we are providing the most seamless service possible. To assess, monitor and report on patient length of stay, delayed transfers of care and practice related issues

The post holder will work collaboratively with leads within the Integrated Discharge Team, Local Authorities, health and social care provider and commissioning services and other external stakeholders, to coordinate and promote effective partnership working at all levels of discharge activity. A high degree of effective coordination, communication, networking and liaison will be required to establish and maintain good working relationships within health and with other agencies locally and nationwide
