Detailed job description and main responsibilities

The Discharge Lounge is very dynamic inpatient area which accepts patients ready to leave hospital, waiting for medications and/or transport home. As a Registered Nurse working in the Lounge you will be expected to:

  • Take on self-initiative to proactively seek for patients suitable to come to the lounge using the TeleTracking system
  • To contact wards to obtain handovers
  • To communicate effectively and professionally with other colleagues and services. 
  • To organize patient's transfers to the Lounge
  • To challenge any gaps in the plans for discharge
  • To complete medication rounds for the patients in the Lounge when they are due
  • To keep patients informed on the progress of their discharge 
  • To liaise with the pharmacy department and teams to follow up on the progress and dispensing of patient's medications 
  • To check patient's TTOs and assure they are complete
  • To liaise with the hospital Social Services/Discharge team, care homes and other hospitals to assure that the patients are discharged to the right places and by the right time.
  • To communicate with NOKs to assure they are kept up to date with the discharge plans and time of patient's leaving hospital. 
  • To Document the care they provide for patients and complete Discharge Checklist for every patient leaving the hospital. 
  • To assure patients receive good care and experience whilst in the Lounge and complete Friends and Family surveys with patients to support that. 
  • To promote team work and maintain good relationships with colleagues  
  • To be able to use the transport booking electronic system and use that to book and track patients transport.
  • To be able to recognize signs of deterioration in patients, complete their observations and escalate appropriately 
  • To liaise closely with the Site practitioners to escalate any issues surrounding patients discharge and maintain safe and effective hospital flow.

The Discharge Lounge will provide you with the opportunity to expand your knowledge and skills in the discharge process and practices which can be very complex at times.

Person specification

Qualifications

Essential criteria
  • RN1/RN2/RN12 or RN Child Registered Nurse on the NMC register
  • Evidence of continuing professional development

Experience

Essential criteria
  • Experience of working on own and in a team
  • Experience of working in the relevant area
  • Experience in Discharging patients
  • Supervision of junior staff

Knowledge

Essential criteria
  • Up to date knowledge of current clinical and professional issues
  • Uses own initiative and is able to take decisions
  • Have knowledge in the different discharge pathways
  • An understanding of Audit and Research Based Nursing Practice
  • Knowledge of the NMC Code of professional standards of practice and behaviour for nurses and midwives
  • Knowledge of Clinical Governance
  • Knowledge of the individual’s responsibility towards health and safety

Skills

Essential criteria
  • Able to assess, plan, implement and evaluate programs of care
  • Is organized and able to prioritise accordingly
  • Ability to administer prescribed medication including transfusion of blood and blood products
  • Basic computer skills including ability to use Microsoft Word and awareness of using TeleTracking system
  • Is able to recognize and escalate deteriorating patient
  • Able to work as an effective team member
  • Able to supervise and teach junior staff
  • Able to manage difficult situations
  • Excellent verbal, written and interpersonal communications skills
