Pathway Co-ordinator and Operational Support

Job overview

·        Take the lead in providing operational business support for the Service Manager’s in Neurology and Stroke

·        Provide full and comprehensive administrative support to the operational team to enable the Service Manager’s to meet the expectation of key performance indicators

·        Provide line management to the administration team working within Medical Day Unit

·        Improve systems and processes in conjunction with the Clinical Pathway Co-Ordinator on Medical Day Unit

Main duties of the job

Pathway Co-Ordinator

·        Take responsibility of the co-ordination of the patient’s pathway to ensure that the patient’s journey is managed efficient, smoothly and in accordance with targets and agreed timescales.

·       Bring appointments forward for patients at are at risk of breaching RTT targets and chase test result with the appropriate department such as Radiology, Nuclear Medicine, Cardiology.

·        Report on long waiters, undated patients and those dated over 18 weeks awaiting diagnostic/results.

·        Liaise with different stakeholders including patients, GP’s and clinical teams.

·        Support the operational team with aspects of service development.

Admin Management

·        To be responsible for the day-to-day line management of Medical Day Unit administrative team, being the first point of contact for all areas of management including performance and conduct.

·    Daily validation of all PTL’s to ensure patient pathways are of good quality and comply with Access Policy standards.

·        Work collaboratively with the clinical tams on each site to plan activity making full use of all available capacity and developing plans to address capacity issues. Ensure agreed actions are delivered and staff accountable for delivering and have appropriate knowledge, skills and training to achieve targets.

Detailed job description and main responsibilities

Pathway Co-Ordinator

·        Take responsibility of the co-ordination of the patient’s pathway to ensure that the patient’s journey is managed efficient, smoothly and in accordance with targets and agreed timescales.

·        Ensure all clinical capacity is identified and capacity fully utilised

·        Identify training opportunities to support and advise the senior medical secretaries in the helping them to meet the targets with outcomes.

·        Cancel, reinstate or suspend clinics on the patient access system and ensure patients are aware of the changes if their appointments are affected

·        Bring appointments forward for patients at are at risk of breaching RTT targets and chase test result with the appropriate department such as Radiology, Nuclear Medicine, Cardiology.

·        Report on long waiters, undated patients and those dated over 18 weeks awaiting diagnostic/results.

·        Liaise with different stakeholders including patients, GP’s and clinical teams.

·        Support the operational team with aspects of service development.

Admin Management

·        To be responsible for the day-to-day line management of Medical Day Unit administrative team, being the first point of contact for all areas of management including performance and conduct.

·        To be responsible for the recruitment to vacant posts and to manage staff performance through annual appraisal and personal development plans.

·        To be responsible for the management of the workload of the specialty administration service, ensuring high levels of quality, turnaround times and targets.

·        Daily validation of all PTL’s to ensure patient pathways are of good quality and comply with Access Policy standards.

·        Work collaboratively with the clinical tams on each site to plan activity making full use of all available capacity and developing plans to address capacity issues. Ensure agreed actions are delivered and staff accountable for delivering and have appropriate knowledge, skills and training to achieve targets.
