Urgent Care Team Leader

We have an exciting opportunity for an Urgent Care Team Leader to join the expanding virtual ward team at KCHFT. 

This post is to support the Virtual Ward model which is a collaboration with the acute trust to deliver hospital standard treatment within a patients’ home. 

The virtual ward philosophy is that care is led by a specialist and provides acute clinical care at home which would otherwise be provided in a hospital for a short duration of up to 14 days.  This community service is nurse led with access to support at the Virtual monitoring Hub.

Prif ddyletswyddau'r swydd

This service is part of the Urgent Care Services which provides a two-hour response to patients in crisis at home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the 
acute hospitals.

Nurses use a wide clinical skills base to provide hospital-level care at home. This includes IV therapy, end of life symptom control and management via syringe driver, catheterisation skills, phlebotomy, 
wound management, risk assessment, as well as general observations and fundamental nursing care.

 

Additional Working Pattern: Long days covering 7 day service

Qualifications

  • Diploma or Degree in Adult Nursing. Registered as fit to practice by the Nursing and Midwifery Council (NMC).

  • Evidence of Specialist qualifications relevant to area of practice (examples non-medical prescribing; mentorship, coaching, clinical expertise) Management or supervisory qualification or experience of study Working towards Master level study

Experience/Knowledge

  • Minimum of 2 years’ experience as Band 5 in area of practice Practical experience in managing health related projects/complex caseloads Supporting others (mentor / educator / supervisor) experience Proficient clinician and clinical role model Insight and understanding of current issues in nursing and the NHS Demonstrates a commitment to professional development

  • Community Experience and working an autonomous practitioner Experience of leading and managing teams Experience with the use of Microsoft Office and a variety of IT packages (E-Pay / E-roster/ AT-P/ RiO)
