Patient Pathway Navigator

We are looking for motivated team members to join our existing Patient Access team to help support an exciting new project.

You will have knowledge of the 18 week RTT pathways, be an excellent communicator, have experience of collating information and have excellent computer skills. The role will involve proactively monitoring, tracking and validating patients on a clinical pathway, ensuring that access targets are met and data held against the patient's pathway is accurate and comprehensive.

Tracking the patients through the referral to treatment pathway. This will involve managing groups of patients through the ‘non-admitted’ and ‘admitted’ element of their treatment pathway to enable treatment to be delivered within key access standards. You will possess the skills and key competencies to support the needs of the department. You will need to be organised, diligent and have patient care as your primary concern. You should have a good understanding of RTT pathway rules as well as awareness and understanding of national targets. You will need good communication skills.

We will offer the successful candidate/s the relevant training, however previous experience of working with Allscripts and RTT pathways is desirable. You will be required to have a professional and mature working attitude, and an ability to liaise with patients and staff.

Qualifications

  • Educated to Secondary Level with 5 GCSE’s at grades A*-C, with 2 of these being GSCE English and Mathematics.
  • NVQ 3 (or equivalent experience).
  • Willingness to undertake additional training.
  • Computer Literate.

  • Knowledge of the e-RS system.

Experience/ Knowledge

  • Have experience of putting together a project/business case and contributed to its implementation.
  • Organisation skills and ability to prioritise work under pressure
  • Working to tight deadlines.

  • Experience of Allscripts and Quattro.
  • Previous experience of working in NHS setting.
  • Good level of understanding of the Government’s access targets for 18 weeks RTT.
  • Experience of data collection, analysis, presentation and report writing.

  • Good Level of IT skills, with the ability to use all Microsoft Office applications, in particular Excel, Access and Word.
  • Ability to work on own initiative and as part of a team.
  • Ability to prepare reports for a wide ranging audience.
  • Excellent written and verbal communication skills.
