Advanced Clinical Practitioner  - Band 8A

£56,388 - £62,785 Inc HCAS per annum pro rata

Full time 37.5 hrs per week (1 weekend in 4 working)

An interesting and exciting opportunity has arisen for a proactive Advanced Clinical Practitioner willing to join our urgent 2 hr Rapid response service within the Urgent Community Response.

The candidate must be autonomous and competent at handling triage and seeing patients in their house to provide safe and effective care based on their advanced skills and consultations. 

The candidate must be a Non-medical prescriber 

Main duties of the job

The post is based within the Rapid Response Team. This service provides a multi-disciplinary urgent assessment and crisis intervention service within a 2 hour response to patients in the community with the aim of preventing hospital admission operating 7 days per week 8am to 8pm.

The post holder is an Advanced Practitioner, acting within their professional boundaries.

The post-holder will be expected to have completed the MSc in Advanced Clinical Practice and be a non-medical prescriber with extensive community experience and skilled to see patients independently, making autonomous decisions in the clinical and diagnostic assessment of patients with undifferentiated and undiagnosed complex health and social care needs to support keeping them in the community and reduce hospital admissions / ED attendance and to develop a comprehensive plan of care and autonomously prescribing as required. 

Referrals are accepted to service from GPs, 111, London Ambulance Service (LAS), care homes and community providers

Qualifications

Essential criteria
  • Master level Award covering and assessing all 4 pillars of advanced clinical practice (clinical practice, leadership and management, education and research
  • Registered non-medical prescriber (if professionally appropriate)
  • Registered with appropriate professional body NMC
  • Undergraduate degree in a suitable discipline
  • Advanced clinical assessment and autonomous practice and clinical decision making
  • Registered with appropriate professional body(NMC, HCPC)
  • Evidence of Continuing Professional Development
Desirable criteria
  • Intravenous cannulation skills. And IVAB therapy administration and management
  • Catheter management skills
  • Experience of working in an Admission Avoidance service

  • Evidence of experience in autonomously managing, practice at an advanced level

  • Car driver with current UK licence and access to a vehicle to use for work purposes
  • Able to function as an advance practitioner with expert clinical skills, critical analysis and risk stratification
  • Able to demonstrate advance skills for assessing and interpreting complex patient conditions, comparing a range of options and taking appropriate action
  • Evidence of the ability to direct and coordinate plans of care working autonomously and collaboratively
  • Evidence of the ability to maintain and monitor high standards of care
  • Advance communication skills, staff, clients other agencies etc.
  • Good understanding of systems in health and social care that are wider than their immediate role
  • Understanding of the principles of clinical governance, particularly in relation to their service area.
  • Evidence of professional development and knowledge.
Desirable criteria
  • Assisting and managing operational risks
  • Intravenous cannulation skills. And IVAB therapy administration and management
  • Competent in venepuncture /phlebotomy

Information Technology

Essential criteria
  • IT Skills to a good level with experience of EMIS, word processing and database management

  • EMIS templates development and search and reporting.
