The role of the band 5 community health nurse is to assist in the provision of child public health services within Medway. The community health nurse will undertake and respond to duty calls for the service, advising families and liaising with other professionals whilst  ensuring good documentation is in place.

  • To work as a member of the health visiting team assisting in the provision of a comprehensive health visiting service to deliver the Healthy Child Programme to an identified population, promoting and preserving the health of individuals, families and communities
  • To undertake and respond to duty calls for the service and ensure good documentation.
  • To lead on public health initiatives as directed by Public Health Commissioners
  • To attend safeguarding meetings when involved in client care

  1.    Communication and relationship skills
  •    Demonstrate highly developed communication and interpersonal skills with clients, colleagues and teams
    •    Ensure effective communication with clients where there may be barriers to acceptance/ understanding e.g. child protection, domestic violence bereavement and where English is not the first language.
    •    Ensure effective communication with all members of multi-disciplinary team and multi agency partners, in particular children’s centres, early years’ providers, social services and GP practices.
    •    Ensure all information about patients is treated confidentially and comply with Trust policies on data protection, information sharing and governance
    •    Participate in team and other meetings as required.
  1.    Knowledge, training and experience
  •    First level registered nurse – RN, RGN
    •    Post registration qualification in a relevant field, e.g. Health Promotion, Child Development.
    •    Highly developed communication and interpersonal skills with clients, colleagues and teams.
    •    Ability to be flexible and develop within the changing needs of the service and clients.
    •    Possessing an up to date Personal Development Plan showing evidence of continuing professional development.
    •    Mentorship qualification or willingness to undertake
    •    To ensure statutory and mandatory training is kept up to date.
  1.    Analytical and judgement skills
  •    Assess, implement and plan services under the guidance of the health visitor
    •    Evaluate individual and community health care programmes in collaboration with the health visitor.
  1.    Planning and organisational skills
  •    Plan and lead the delivery of designated public health and health promotion activities
    •    Plan individual and community health care programmes
    •    Responsible for  planning own work
    •    To ensure statutory and mandatory training is up to date in line with organisational policy.
  1.    Physical skills
  •    Keyboard skills
    •    Ability to travel to diverse sites in a timely manner.
  1.    Responsibility for client care
  •    To work in collaboration with the Health Visitor in planning, implementing and evaluating individual and Community Health Care Programmes.
    •    To lead the delivery of designated public health and health promotion activities.
    •    To assess, implement and plan services with individual families under the guidance of the Health Visitor.
    •    To act as an infection control link/ resource to the team 
    •    Safeguard the client group by adhering to child and adult protection guidelines and procedures.
    •    Ensure effective communication with clients and colleagues
    •    To ensure the provision of a high quality service that is led by client need and is evidence based.
    •    Access clinical supervision as per Trust Policy.
  1.    Responsibility for policy and service development implementation
  •    To support the Health Visiting Team in service and practice development initiatives to improve standards and outcomes.         

    8.    Responsibilities for financial and physical resources
  •    Comply with the Trusts Standing Orders and Standing Financial instructions
    •    Responsible for the safe and careful use of equipment in clinics and during home visits (e.g. weighing scales).
  1.    Responsibilities for human resources (HR)
  •    Line management of band 4 and below
    •    Participate in mentorship and education of qualified and unqualified staff gaining community experience
    •    To provide support and advice to other colleagues within the team.
  1.    Responsibilities for information resources
  •    Responsibility for timely accurate record keeping and safeguarding client confidentiality by adhering to Caldicott Principles.
    •    Record and submit accurate and timely data as required to inform service planning, management and evaluation.
  1.    Responsibilities for research and development (R&D)
  •    Completion of audits as required by the service.
  1.    Freedom to act
  •    Qualified nurse who uses own professional judgement. Able to work alone and unsupervised under the management of a qualified health visitor
    •    Adheres to the NMC Code of Professional conduct.
