Cardiac Assistant Practitioner

Job overview

We are looking for a Cardiac Assistant Practitioner to join our East Kent Community Cardiac Nursing Team based in Whitstable. 

We provide a specialist service to adult patients with 
complex cardiac conditions across East Kent, primarily heart failure. As part of the highly specialist nursing team, you will help support patients with chronic cardiac conditions that have been identified as having 
ongoing needs and would benefit from focused management.

Main duties of the job

Whilst patients are predominately seen in the clinic setting there is the ability to see patients in their homes 
or digitally where clinically indicated. You will undertake the assessment, delivery and evaluation of care 
required for this patient group in keeping with your role and competencies, and under the supervision of the 
specialist nurses. Provide the necessary health education and advice to support the patient in manging their 
cardiac condition. 

We have strong links with our community based cardiac rehabilitation team. We work closely across the 
boundaries of primary and secondary care whilst ensuring a seamless service for our patients on a cardiac 
pathway of care. We have access to weekly multidisciplinary team meetings with a consultant cardiologist

Qualifications

Essential criteria
  • Education to GCSE level or equivalent Able to demonstrate competence in maths and English NVQ Level 2 OR 3 and relevant experience OR NVQ Level 4
  • Valid driving licence in the UK
Desirable criteria
  • ECG

Experience

Essential criteria
  • Minimum of 2 years’ experience as a Senior Healthcare Assistant / Worker in a health or social care setting
Desirable criteria
  • Experience - cardiac or community

Skills and abilities

Essential criteria
  • Sound communication and interpersonal skills (verbal and written)
Desirable criteria
  • Recording an ECG
