Are you a compassionate and aspirational Pharmacy Technician with a passion for delivering excellent patient-centred care? A fantastic opportunity has presented for an enthusiastic and committed individual to gain specialised skills in a clinical role within our friendly and supportive team.

You will be responsible for delivering a high standard of clinical care to patients at our hospital(s) through the provision of a safe and effective clinical medicines optimisation service. 

You will be also be a key member of the multidisciplinary ward team supporting safe discharge of our patients which includes completing medicines management assessments, supporting self administration of medicines and providing counselling on medications plus much more.

In this progressive role, you will be fully supported to gain the skills and experience required to practice at an advanced level in the community hospital setting, with the additional benefit of not working weekends or bank holidays!

You need to be a qualified Pharmacy Technician registered with the GPhC to apply, however, applications from candidates due to qualify & register this year will be considered and we welcome applications from Pharmacy Technician colleagues working in all sectors of the profession as hospital experience is not essential as you will be fully supported to gain the knowledge, skills and experience required.

Prif ddyletswyddau'r swydd

  • You will provide a comprehensive medicine management service to the community Hospital wards. This will include focusing on medicines reconciliation on admission, discharge facilitation, patient counselling
  • Working closely with the multidisciplinary team on the ward is a key aspect of this clinical post. As a regular attendee at Multidisciplinary team meetings and board rounds, you will input directly into patient care to support safe discharge.
  •  You will predominantly work at your base hospital but with the opportunity to travel to different KCHFT hospital sites
  • You will work with a team of skilled Pharmacists and Pharmacy Technicians to maintain and develop an excellent and progressive pharmacy service in line with the NHS long term plan and NICE.
  •  You will be fully supported with your training and development and benefit from the wealth of training courses available to NHS Pharmacy team, including both clinical and management, and leadership opportunities.
  •  Pharmacy services are highly regarded by both patients and Trust staff and the high profile of our service and the positive feedback we receive make it a stimulating and rewarding place to work.
  •  The post does not include participation in weekend, Bank Holiday and late duty rotas

Qualifications

  • Regionally Accredited qualification in Medicines Management at Intermediate Level or historical equivalent (Regionally Accredited Consultation Skills plus Medicines Management Skills). Practice supervisors’ course or equivalent.

Experience 2

  • Experience of working within a multidisciplinary team
  • Experience in performing audits and participating in projects pertaining to the development and maintenance of service improvements

  • Able to problem solve by obtaining relevant information and making decisions based on the information.
  • Demonstrate practical knowledge and experience in Microsoft Office products, internet systems

  • Specialist knowledge, including clinical knowledge, gained through training and experience – knowledge of pharmaceutical care of patients in Intermediate, Palliative Care, Respite Care and Continuing Care.
  • Experience and knowledge of the Supply, Distribution, Storage, Administration and Disposal of Pharmaceuticals in Community Services

  • Able to work independently and on own initiative
