KCHFT and ABC PCN are working collaboratively to offer an innovative opportunity for two pharmacists to develop patient centred medicines optimisation within each setting. You will be employed
This exciting opportunity will require, cross sector multidisciplinary working both with frail elderly patients within our Community Hospitals and within the GP Practices. You will be responsible for providing safe and effective medicines optimisation
For those band 6 or newly qualified Pharmacists who require more support we are able to offer the right applicant a Band 6 to 7 progressive post. We will support successful candidates through the Post Graduate Clinical Diploma, Independent Prescribing and CPPE Primary Education Pathway. If
Main duties of the job
You will develop an expertise in the optimisation of medicines in frailty and other chronic conditions, working with multidisciplinary teams to ensure that we achieve the best medicines outcomes for patients. This is an evolving role and we are looking for enthusiastic
