Job overview

An exciting opportunity has arisen for a community rehabilitation band 6 Occupational Therapist  to join a supportive and enthusiastic community therapy service,  working across community delivering rehabilitation in patients own homes or current place of residence, following discharge from an acute bed, or supporting long term condition management.

You will be working with patients with a large variety of conditions and disabilities. You will be responsible for assessment, intervention, and discharge of a caseload of patients, act as the direct supervisor to staff and provide support and expertise to the other members of the team.

We offer a fabulous training programme within a supportive team.

The post-holder will be required to rotate across all therapy teams in the Community and Intermediate Care division to support clinical and workforce development.

The service operates across 6 days.

Main duties of the job

The post holder is an autonomous practitioner, responsible for provision of specialised assessment and treatment  of patients referred to the service. Responsible for managing a caseload of patients with a variety of complex conditions including long term condition management and acute hospital discharges. You will see patients in various settings, working as part of an MDT and liaising with external stakeholders. You will carry out specialist assessment and treatment including the prescription of essential equipment as part of the therapeutic intervention.

You will play an important link with PCN and MDT teams in attending meetings as part of neighbourhood team working.

Car driver with access to a vehicle for work purposes is essential.

6 day working is in effect - Monday- Saturday and bank holidays on a rota basis.

  • o Post graduate therapy experience to include community and rehab

  • o Profession specific Diploma/degree in Occupational Therapy or Physiotherapy o Health Professions Council Registration o Evidence of CPD maintained in a portfolio including attendance at recent postgraduate courses relevant to the clinical fields in rotations. o Membership of the Chartered Society of Physiotherapy/ Royal College of Occupational Therapy o Involvement in Special Clinical Interest Group
