This post will be based at ELHT with the main purpose of leading the HPB cancer nursing service. 

The Lead CNS role will:

Liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries

Work autonomously to deliver specialist clinical care appropriate to the needs of the patient group and based on principles that are research based and enhance the quality of patient care

Undertake nurse led clinics and to be responsible for excellent standards of nursing care to patients through the planning, co-ordinating, delivering and evaluating nurse-led clinics and services

Develop a nurse-led service and pathway for pancreatic cysts

Line manage clinical nurse specialists within speciality

Lead and support with specialist nursing knowledge the development, and implementation of programmes of nursing care for individual patients and the client group as a whole

Contribute to the facilitation of the planning, delivery and evaluation of care pathways for a designated group of patients

Contribute to training and education within specialist area

Promote an innovative and progressive attitude to the continual improvement of patient care through reviewing the evidence base and embedding this in practice through engagement in research activities

Act as a visible role model and expert practitioner

To lead on the provision of a clear focus for nursing clinical leadership across the HPB nursing service and to be responsible for ensuring the delivery of high quality patient care

Draw on a diverse range of knowledge in their decision-making to determine evidence-based therapeutic interventions

 

To be responsible and accountable for the provision of expert and specialised care, in collaboration with the specialist MDT

To be responsible for the implementation of audit, quality and risk management initiatives and support the evaluation of the HPB nursing service including patient experience

To maintain a patient-centred approach when planning care, involving patients, carers and members of the MDT in care decisions

 

Ensure quality is central to clinical practice by delivering evidenced based individualised care, through holistic needs assessment, planning and evaluation of care promoting personalised at all times

 

Support patients in the delivery of care and meeting their health and wellbeing needs by providing expert advice and information, promoting their wishes and beliefs and addressing their concerns

 

To lead on the development and monitoring of referral pathways, protocols, policies and guidelines with particular relevance to the HPB service

To ensure that relevant robust data is collected which can be used to evaluate and improve care and service delivery

 

Review and evaluate services collaboratively with own and other professions and users, identifying areas for service development and acting on this accordingly, making best use of resources, improve practice and health outcomes in line with local and national best practice.

 

To lead on the identification of service risk/s and act in accordance with the relevant policies and procedures

 

To ensure completion of clinical incident reporting where an adverse incident is identified which has caused harm or has the potential to cause harm to a patient, staff member or any individual

 

Participate in developing a shared vision of the service and work with the multi-disciplinary team, organisation and external agencies to achieve this

Lead on the development and implementation of national policy and practice initiatives and to implement these at a local level within the speciality

 

Develop evidence based standards, policies and guidelines at a local, network and national level to improve the practice of own and other professions

 

Provide professional, clinical and managerial leadership to nursing and associated staff, and ensure the provision of a high quality, responsive nursing service.

 

Engage in the implementation of improvements in nursing practices within own areas of responsibility in line with local and national agenda and patient feedback.

Collaborative working with CNS team across the Cancer Alliance and developing a HPB working group, to improve the patient pathway as the tertiary centre for HPB within the alliance

 

Collaboration with the HPB RDS CNS to ensure a smooth transition from the outpatient and inpatient settings through robust effective planning and collaboration, co-ordinating continuing care and evaluating effectiveness

 

To lead, negotiate, supervise, participate in, and manage caseload of patients within nurse led clinics. Provide professional, clinical and managerial leadership to nursing and associated staff, and ensure the provision of a high quality, responsive nursing service.

Collaborate on all aspects of quality management related to the nursing and clinical area.

Lead the implementation of improvements in nursing practices within own areas of responsibility in line with local and national agenda and patient feedback.

To recognise the limits of own clinical knowledge and skills and to seek medical support whenever needed to provide safe, effective and timely patient care.

To contribute to the investigation of complaints and untoward incidents and ensure recommendations and related education/training programmes/action plans are implemented.

To promote new and innovative forms of nursing practice, backed up by relevant education and training plans, to deliver care that enhances access to the department and specialist services

Lead the nursing team and be responsible for the direct line management of team members

  • Registered General Nurse
  • First level degree in related subject, or undertaking degree pathway
  • Advanced communications skills training, or prepared to work towards
  • Evidence of recent continued professional development
  • Non-medical prescribing
  • Clinical examination/physical assessment course/module
  • Experience of undertaking nurse led clinics
  • Experience of working autonomously and across multiple teams
  • Ability to assess and interpret specialist clinical information relating to a patient’s condition and diagnostic investigations
  • Leadership and motivational skills
