Job overview

The Children’s Community Nurse (CCN) (Reactive Care Team) provide the  evidenced-based nursing care and support for children with a wide range of conditions predominantly caring for children with Acute Childhood illness and wound care. 

The role is integral to the wider Integrated Community Paediatric Service, which is supported by a large team of experienced, senior community paediatric nurses and paediatrician’s.

The post holder may be required to rotate across all areas of the Integrated Community Paediatric Service and Paediatric inpatient areas.

Main duties of the job

To deliver evidence based nursing care to children and young people (CYP) in a variety of community settings by providing clinical assessment, planning, delivery, evaluation of care and clinical advice in liaison with the team leader, senior staff, peers and or multi-disciplinary professionals.

The Children’s Community Nurse will provide support, evidenced-based care and advocacy for children with a wide range of conditions such as acute and long term and life limiting conditions. Children and families will be cared for within the home and various other ambulatory care settings such as nurse led clinics, children’s centres and schools. The role is integral to the wider generic Children’s Community Nursing Team (CCNT).


The post holder may be required to rotate into the acute Trust and to work within the different teams within the CCNT to maintain/further develop knowledge and skills that promote ‘Care Closer to Home’. This will enable staff to have the required elements to care for the children of Bolton with acute, long term and continuing care conditions.

The post holder will be required to act in a manner consistent with the NMC Code of Professional Conduct, carrying out their duties in accordance with Trust policies and procedures.

Detailed job description and main responsibilities

To recognise and monitor acute exacerbations of illness and initiate appropriate nursing support and action any urgent medical interventions i.e. senior support, referral to urgent care.

Be flexible and committed to supporting the wider CCNT when service requires, for care of patients with long term conditions, chronic conditions such as Eczema, Asthma and Constipation, to work alongside lead/Senior nurses in the implementation of care plans and delivery of health promotion.

Work collaboratively with the hospital inpatient areas, accident and emergency, A&E, Observation and Assessment unit and primary care to facilitate early and prompt discharge and deflect unnecessary admissions to secondary care. With Senior supervision, nurse to provide support for ambulatory care service to promote CCNT referrals.

Collaborate with the multidisciplinary team and other service providers to ensure care is patient centred, efficient and effective and to develop guidelines and policies for children with acute and continuing health care needs.

Proactively safeguard the welfare of all CYP and adults and actively adhere to safeguarding procedures and policies, attend training to achieve Safeguarding Level 3 and work in partnership with other health and social partners. Participate in all relevant safeguarding meetings relating to CYP at risk.

 Administer prescribed medication safely in accordance with the Trust Medicines Management policy and within the guidelines and protocols of the Integrated Community Paediatric Service.
