Band 4 Nurse Associate -Pre Op Assessment 

Part time 19 hours per week. Permanent post.

An opportunity has arisen to recruit an enthusiastic, motivated and flexible individual who are able to work independently within the Pre Op Assessment Department.

Experience in Pre Op Assessment is  not essential but would be an advantage . The post holders will be involved in assessing patients from Multiple specialties who are due to come in for surgery. You will need to be comfortable working both independently and part of the Pre Op Assessment team as you will be responsible for your own cohort of patients.

Prif ddyletswyddau'r swydd

The Pre-Op Assessment Nurse works unsupervised, within their scope of professional practice to deliver appropriate nurse led face to face or telephone Pre-Assessment for all minor and intermediate elective Surgery patients (ASA 1 to ASA3)

  • The Nurse works within clearly defined protocols and professional guidelines. 
  • The purpose is to make a clinical decision determining a person’s fitness to attend for surgery and / or Anaesthetic as a result of that assessment.
  • The Nurse manages their own caseload in the form of a clinic.
  • Request clinical investigations and acts upon the results e.g. blood tests, ECG’s MRSA swabs
  • Undertakes baseline observations 
  • Undertakes phlebotomy, Glucometer readings, MRSA swabs and ECG investigations as required
  • Plans and implements appropriate care following the assessment
  • Frequently required to assist patients with moving and handling 
  • Undertakes the onward referral of patients following assessment if required
  • Writes reports, record observations and maintain patient records which detail actions and decisions made
  • Keep patients and carers informed when appropriate, maintaining patient confidentiality
  • Maintains and is proactive in engaging with patients and carers  and in avoiding or resolving complaints
  • Collaborate with the multi-disciplinary team and other service providers; to ensure care is patient centred, effective and efficient
  • Support Junior medical staff in Doctor led and/or anaesthetics clinics

Communication

  • Ensure patients are fully informed of planned care and next steps and how to contact service if required

 

  • Attend departmental meetings as required, to ensure an awareness of developments within the service.

 

  • Attend and be an active participant in regular clinical team meetings, contributing to the assessment and planning of patient care.

 

  • Communicate, as appropriate, with primary care, to inform of patient health problems and recommendations for clinical management and further care planning.

 

  • Assist in maintaining an effective communication network, both written and verbal, informing senior staff of all matters outside your role/sphere of competence; cascading information to other staff as required.

 

  • Liaise with other service providers to support the efficiency of the service.

 

  • Be proactive in establishing positive and professional communication in relationships with the public.

 

Organisation and management

  • Assist in the development and implementation of protocols for pre-assessment.

 

  • In the absence of the manager, ensure equipment/facilities are maintained and faults corrected.
  • Maximise the use of manpower and resources and promote new ways of working.

 

  • Report and record all untoward incidents and contribute to investigations, as needed.

 

  • Undertake delegated tasks as requested by the Manager.

 

  • Develop and maintain an area of special interest, to contribute to the expertise of the team, and assist in developing ongoing programmes of improvement for the preoperative assessment service

 

 

Staff Development

  • Assist in the delivery of effective induction programmes for staff new to the area.

 

  • Participate in appraisal and develop an annual Personal Development Plan to identify training and development needs, in line with personal and organisational objectives.

 

  • Develop and maintain an area of special interest, to contribute to the expertise of the team, and the development of ongoing programmes of improvement.

 

Evidence Based Care, Quality and Standards

  • Keep up to date with current trends within speciality and ensure practice is evidence based.  In discussion with the line manager and professional lead, promote changes in care/activity, where evidence identifies the need.

 

  • Ensure documentation on untoward incidents is completed, and produce reports as requested by line managers.  

 

  • Take part in Clinical Audit review.

 

  • Comply with, and contribute to Clinical Governance initiatives, CNST, and Research and Development initiatives.

 

  • Maintain a professional, helpful and customer focus approach to staff, patients, visitors and colleagues.  Treat all equally; and with dignity and respect.

 

  • Promote the use of Information Technology, to improve and inform practice.

 

LEGAL & PROFESSIONAL RESPONSIBILITIES

 

  • Adhere to Trust and departmental policies and procedures.

 

  • Maintain a safe environment for yourself and others, taking reasonable care to avoid injury. Co-operate with the Trust to meet statutory requirements.

  • All employees have a duty to report any accidents, complaints, defects in equipment, near misses and untoward incidents, following Trust procedure.

 

  • To ensure that Health and Safety legislation is complied with at all times, including COSHH, Workplace Risk Assessment and Control of Infection.

 

Confidentiality:

 

  • Working within the trust you may gain knowledge of confidential matters which may include personal and medical information about patients and staff.  Such information must be considered strictly confidential and must not be discussed or disclosed.  Failure to observe this confidentiality could lead to disciplinary action being taken against you.
