Job overview

Are you an experienced Nurse who is interested in a community role, visiting and supporting patients with long term conditions in their own homes?  If so this role could be for you.  This opportunity is ideal for a proactive and enthusiastic nurse to support the Community Matron to manage the defined Matron caseload and as part of the integrated team you will maximise each others skills and knowledge for the patients benefit.

The current postholder is retiring and therefore this post would not be available until 27th December, 2024

 

Main duties of the job

You will expected to support holistic and clinical assessments in order to meet the patients needs, providing education regarding how to manage  their long term condition whilst also working alongside a team of other professionals (District Nurses, Community Therapists, Integrated Care Coordinators, Health and Wellbeing support Workers etc within Fylde Enhanced Primary Care Team). 

The role will include building good relationships and working closely with GP's to ensure seamless, effective and timely care. 

In Fylde we are proud to be a leader in integrated working which will offer you the chance to work with Nurses and other health professionals within a highly skilled multidisciplinary team alongside the GP provided services. You will be supported  to develop the skills to undertake holistic assessments and care provision to patients in their own homes.

Detailed job description and main responsibilities

Support the Community Matron with the assessment, planning, implementation and evaluation of the individual requirements of patients and carers on the Community Matron caseload

Provide a high standard of evidence based nursing care to patients in the Community.

Use a high level of communication skills with patients and their families, including those with English as a second language or learning disabilities, to communicate condition-related

Actively seek to promote health both individually and to patients and carers.

Essential criteria
  • Registered General Nurse with NMC Current registration
  • Post registration training in specialty area of Long-term conditions i.e. COPD, diabetes, heart disease, asthma.
Desirable criteria
  • Training in risk management

Experience

Essential criteria
  • 12 months community experience Experience in Long term conditions
  • Post graduate experience working with a multidisciplinary team.
  • Experience of working with patients with a variety of chronic disabling or life limiting illnesses.
  • Experience of delivering specialty training to colleagues in health and/or other agencies
  • Knowledge of current best practice
  • Ability to apply knowledge and principles of clinical Governance
