Job overview

This post is for an Advanced Clinical Practitioner. The vacancy is only open to those who are either a current student on this course or already have the MSc in Advanced Clinical Practice (in both cases, evidence will be required during pre-employment checks). Students will commence at Band 7 and once the MSc is awarded, will be progressed to Band 8A.

An Enhanced DBS (with Adults Barred List check) will be obtained for the successful candidate during pre-employment checks. 

This is a permanent full time (37.5 hours) across 7 days between service hours of 0800-2200 (including bank holidays).

Main duties of the job

We are looking to recruit an ACP to work within the Community React Team (CRT). The post holders will work as part of the Community multi-disciplinary teams to manage patients in the community setting, including patients' home, care homes and the SDEC ambulatory chairs at the acute hospital site. The post holders will support the clinical team in terms of clinical leadership and expertise. 

The CRT works in collaboration with North West Ambulance Service (NWAS), Primary Care, Care Homes and the wider healthcare economy, with a focus on safe admission avoidance.

This is an exciting time for the Community React Team as we continue to evolve, working closely with our key stakeholders. The expectation is to work within a changing environment and to drive the transformation agenda.

You must be able to travel across Trust independently.

The role will require work across all areas of the Trust, including those designated as positive, asymptomatic and negative for COVID.

Detailed job description and main responsibilities

Professional & Clinical Practice

  • Ensure clinical practice reflects all current NMC documents including (a) Code of professional Conduct (b) Standards for record keeping (c) Administration of Medicines.
  • Work within the trust’ s policies and procedures and the parameters set down in agreed local clinical protocols and guidelines and national best practice guidelines such as those published by NICE and within own sphere of competence. Trust policies, procedures and guidelines can be found on the trust website.
  • Assess a range of patients with undifferentiated and differentiated problems and under supervision if needed and work towards the development of a comprehensive management plan in undifferentiated and differentiated presentations.
  • Develop and utilise advanced knowledge and skills to assess, examine, diagnose, plan, treat and refer patients if needed to meet the patients physiological, psychological and social needs and in response to the changing needs of the service.
  • Utilise appropriate investigations to confirm diagnosis and action as needed.
  • Work towards organising the clinical management of patients, utilising advanced knowledge and skills prioritising patients accordingly. This may be through triage, face to face and telephone consultations, MDT meetings as appropriate using professional clinical judgement.
  • Recognise and respond appropriately to urgent and emergency situations with the support and guidance from other team members.
  • Work towards managing situations that are unfamiliar, complex or unpredictable and seeking to build confidence in others with the support and guidance from other team members. This will involve utilising clinical judgement and decision making to initiate appropriate interventions and treatment for patients.
  • Make and receive referrals to and from other health care professionals
  • Communicate patient’s condition, verbally and in writing as required, to other members of the multi-professional and multi-agency team and contribute towards the decision-making processes in patient management.
  • Discuss with mentor / supervisor the utilization of medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice.
  • Work towards completion of the Non- Medical Prescribing qualification (V300) as agreed in the MSc Advanced Practice. .
  • Support and promote patient independence and daily living within their home.
  • Support patients to make healthy lifestyle choices through implementing health education and promotion strategies and promote self-care where appropriate
  • Share with the employer the responsibility to achieve the MSc in Advanced Practice by matching organisational vision / aims with personal objectives by attending / participating in regular appraisals as appropriate.
  • Have regular contact with the named mentor, clinical supervisor to support individual learning, develop expertise in advanced clinical decision making, leadership, research and audit skills. Training will be supported through the integration of theory and practice.

Evaluation & Research

  • Facilitate the development of clinical practice through research and education by integrating theory and practice.
  • Implement research-based practice and assist in disseminating research findings and support changes in care.
  • Engage in research activity and audit as appropriate, in order to enhance knowledge of the role and service delivery and / or improve patient outcomes.
  • Support and facilitate other staff if appropriate, who are undertaking research / audit projects.
  • Facilitate collaborative links between clinical practice and research through proactive engagement, networking with academic, clinical and other active researchers.
  • Apply new knowledge to the development of own practice demonstrating evidence- based care.

 Responsibility for Health & Safety

Compliance with the Health & Safety at Work Act 1974 – the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
