Job overview

South Lakes Community Mental Health Team (CMHT) are looking to recruit a qualified Band 6 Mental Health Nurse/Occupational Therapist or Social Worker. The applicant would need to be registered with a registered body. This is a permanent 37.5 hour contract.

The team serves the population of the South Lakes area. It is an established team with a friendly and supportive culture. There is a visible and supportive leadership team who are committed to clinical supervision and professional development.

This is a demanding role in a busy service. Ideally, you will have current or previous experience in a community setting and be able to work effectively in a multi professional context. You should have a collaborative and enthusiastic approach as well as passion and commitment in developing and promoting recovery based approaches for adults with severe and enduring mental health needs.

The post requires that you are a good communicator, and are able to adopt a flexible, adaptable and proactive approach to your work.

You will benefit from encouragement to continue to develop your own specialist knowledge and competence. Your development will be guided using clinical supervision, management supervision, Team/Peer support, internal and external training opportunities and the Trust’s appraisal process.

The service is based at Kirkstone house in Kendal. The service is operational between the hours of 9.00am and 5pm however some out of hours work may be necessary to meet service demands

Main duties of the job

  • Provide comprehensive assessment, advice and treatment to people with highly complex needs referred to Community Services in accordance with care pathways for given specialism.
  • Deliver specialist interventions in accordance with care pathways for given specialism.
  • Develop packages of individual care and treatment and according to identified need within the Care Co-Ordination and Care Act Framework.
  • Work closely with other professionals within and outside the Community Team.
  • Actively support the Team Leader/Advanced Practitioner in the achievement of KPI / CQUIN and related County Council targets, and maintenance of registration with regulating bodies
  • Provide supervision and direction to practitioners and support staff within community services, undertaking appraisals and supporting the personal development of colleagues

The post holder will have a positive attitude to the care of the service users, be adaptable and resourceful and will be required to work regularly without direct supervision.

They will perform a range of roles including acting as Care Co-Ordinator for “highly complex” service users and thereby liaising with general practitioners and other service providers to ensure appropriate care, treatment and support is available to individuals in line with their care plan.

The post holder must be prepared to work across service lines/specialism/network according to the needs of the service
