SCF in Geriatric Medicine (with Special Focus on Surgical Liaison)

NHS Medical & Dental: Senior Clinical Fellow

Job overview

We are looking for a hardworking and motivated colleague who is keen to learn how to care for frail older trauma patients. This role would particularly suit doctors who have completed IMT-training (or equivalent) who are looking to progress to specialty training in Geriatric Medicine and also those who wish to go down the non-consultant career grade or SAS route.

This post will be for a period of 1-2 years (with review after the first year) and can be flexible i.e. full time or part time.

The appointee (you) will be trained in Comprehensive Geriatric Assessment (CGA) and provide clinical input to patients admitted under the Major Trauma pathway under the clinical supervision of the existing Consultant Geriatricians. You will work with surgical teams from across a range of surgical specialties to enhance the quality of care provided to elderly patients who present with traumatic injuries.

Main duties of the job

You will:

- Work at registrar level within the Elderly Medicine department at the Royal Preston Hospital. You will work with the Consultant Geriatricians who will provide clinical and educational supervision.

- You will be expected to see new patients admitted under the Major Trauma pathway to provide CGA.

- You will work with your supervisor to develop a proactive service for proactively reviewing these patients to ensure rapid recovery, early recognition and treatment of complications, and proactive discharge planning.

- You will develop experience in general medicine and elderly medicine (CGA, frailty, the geriatric syndromes, osteoporosis management and advance care planning).

- You will develop skills in communication (including obtaining quality collateral histories and advance care planning) and participate in multidisciplinary team working.

- You will attend (and contribute) at the Major Trauma service governance meetings.

- You will have opportunities to lead a service development or quality improvement project and work with our continuous improvement team to measure your success.

- There is an option of participating on the general medicine middle-grade (registrar) on-call rota if you wish (optional).

Working for our organisation

Lancashire Teaching Hospitals NHS Foundation Trust forms one of the major acute Trusts in the North West of England with all the main specialties. We are the tertiary centre for major trauma, neuro-sciences, renal, oncology and plastic surgery/burns for Lancashire and South Cumbria. All our trauma work occurs at the Royal Preston Hospital site.

The development and expansion of the Elderly Medicine and Major Trauma services at Lancashire Teaching Hospitals is very high on the Trust’s agenda. We care for around 600 trauma patients per year, around 250 of which are classed as having major trauma. This equates to about 2 new patients per day and approximately 20-30 inpatients on our trauma wards on any given day.  Our existing consultant geriatricians currently provide 3.5 sessions/week to Major Trauma patients, however we wish to further develop this service.
