Job overview

This is a great opportunity to broaden your skills and knowledge and gain experience within (Medicine) Renal Transplant. You will work closely with patients on every step of their Transplant journey. Involving the traditional nursing role, you will be responsible for the education and coordination of patients care, whilst working alongside your Transplant team of nurses and Consultants. As we expand the post-transplant service, the vision is to bring accessible care to the northern parts of our catchment area which would include Kendal/Clifton/Ulverston, occasional travel may be included within this role.

You will be working in a well-supported team with support from the senior nurses and Matron. Main duties of the job

The duties and responsibilities listed within the job description should be undertaken in accordance with the levels of competence as defined in the KSF outline for this post.  In addition all staff are expected to act in accordance with the values and behaviours of the Trust

 Main duties include, Assess, plan, implement and evaluate care to support CKD patients’ needs who may or have received a kidney or kidney /pancreas transplant in accordance with agreed local and national standards and evidence based guidelines, as part of the Renal Transplant Team.

Detailed job description and main responsibilities

  • Assess, plan, implement and evaluate care to meet patients’ needs in accordance with agreed local and national standards and evidence-based guidelines.
  • Support and provide patient education and health promotion.
  • Be responsible for the delivery and co-ordination of care, through appropriate professional practice, delegation and supervision of duties carried out by junior staff.
  • Maintain accurate and complete records of all care activities, in a timely manner, adhering to local and professional standards for documentation.
  • Ensures environment of care is safe, well equipped, and compliant with standards for hygiene and cleanliness. 
  • Act as a role model, presenting a positive image of self, team, and the organisation.

Person specification

Qualifications & Education

Essential criteria
  • Appropriate NMC registration
  • Diploma in Nursing
  • NMC recognised mentorship qualification or willingness to work towards 18 months commencement in post
  • Evidence of continuing education or development
Desirable criteria
  • Degree

Knowledge & Experience

Essential criteria
  • Evidence of clinical competence in area of speciality e.g. Renal / Transplant
  • Awareness of changes within Nursing practice
  • Knowledge of clinical governance and the relevance to practice
  • Knowledge of relevant policies and procedures
  • Demonstrates competency during internship
Desirable criteria
  • Knowledge of ethical issues
  • Knowledge of leadership and management
  • Research and development
  • Experience of mentorship and ability to identify learning and development needs
