An exciting opportunity has arisen for a Band 6 Neonatal Homecare Nurse to work within Neonatal Homecare Services, hosted by Leicester hospitals.
This model of Neonatal Community Care is provided across Leicestershire and Rutland and the Northamptonshire region, including University Hospitals of Leicester, Kettering General and Northampton General Hospitals . Posts currently available for the Leicestershire area.
The post holder will be an excellent team worker and will be responsible for ensuring the highest standard of clinical care and evidence based practice for infants within the scope of neonatal outreach, providing specialist on-going care for the neonate and advice, support and education to the family in the home environment.
Substantive contracts would be with UHL , honorary contracts with Kettering and Northampton . We have posts available, with a main base of Leicestershire covering both Leicester hospitals. Cross cover may be required on occasion at Kettering and Northampton.
We are delighted to now offer a 7 day service at each site and have recently increased services available to families with the new launch of a Home phototherapy service for jaundiced babies.

The Neonatal Home care Team provides a service covering 7 days a week, from 08:00-1700 Monday to Sunday
This service provides continuing community care for babies and their families in the home setting. Home nasogastric tube feeding and the Home oxygen service is currently well imbedded , and more recently we have expanded services further to include care of babies at home on phototherapy for the treatment of jaundice.
We are seeking highly motivated Neonatal or Paediatric Nurse's to join our forward thinking family focused neonatal team. The successful candidate will demonstrate the drive and motivation to support the implementation of such an exciting service . To be successful in this role you will need to be registered nurse/midwife with post specialist neonatal course QIS experience, current Health visitor or Paediatric community care experience would also be considered

- The job holder will assist Neonatal Unit, transitional care based staff with early and appropriate discharge planning in accordance with the criteria for outreach care.
- To provide a seamless timely discharge from hospital to the home environment.
- The job holder will carry continuing responsibility for a defined caseload, including the assessment of care needs and the development, implementation and evaluation of complex care programmes, ensuring continuous provision of integrated care.
- To provide on-going holistic care within the community setting for vulnerable neonates and their family.
- Provide specialist on-going care for the neonate and education, advice and support to the family in the home environment.
- The job holder will take a proactive role in the education and training of the multidisciplinary team(s), providing on-going supervision and support as an expert.
- Resource for all disciplines where neonates require on- going care needs within neonatal outreach.
- To work effectively and efficiently as an Outreach team member as well as independently in the planning, assessment, implementation and evaluation of neonatal community care.
- To plan, time manage and initiate a weekly schedule based on workload requirements, which will meet the needs of the service and ensure that individual family needs are met.
- To be confident and competent within the Outreach setting as a lone worker in accordance with trust policy.
