This is an exciting opportunity to join Leicester Hospitals in a newly funded Advanced Pharmacist Practitioner Frailty role. Given this is new to the organisation, you will be working closely with key stakeholders across several clinical areas, patient safety and primary care on both trust and system wide initiatives. Key contacts will be our Frailty and Geriatric Medical and Pharmacy teams.

 

An independent prescribing qualification will be required for the role, or a commitment to work towards one. Key to this role is collaborative working and excellent communication skills, good insight into clinical governance related to frailty and pragmatic solutions to complex problems.

This role is an 8b Advanced Pharmacist Practitioner post and will have a bespoke job plan.

 

 

Main duties of the job

  • The post holder will be responsible for the provision of a highly specialised pharmacy service to patients across the Trust.

  • They will be expected to provide expertise to nursing and medical staff (within the Trust and externally) on the management of individual patients with frailty. 

  • They will be expected to integrate with the relevant clinical teams to ensure efficient transfer of clinical and medicines management issues.

  • They will take overarching responsibility for all pharmacy research and audit projects within the service and be responsible for overseeing training including undergraduate placements, trainee pharmacists and diploma including independent prescribing.

Detailed job description and main responsibilities

Responsible for the provision of the Frailty service to UHL.

Specific responsibilities include:

  • To ensure the provision of high quality, safe, comprehensive (clinical, supply, financial and operational) pharmaceutical service to the Trust.
  • If a prescriber, to undertake clinics within a specific area
  • Clinically screen prescriptions
  • Provide expert clinical advice
  • Oversee training of staff, including undergraduate placements, trainee and foundation pharmacists, diploma and IP students

Qualifications

Essential criteria
  • Registered with GPhC
  • Member of professional organisation e.g. RPS, BGS or UKCPA
  • Registered practicing Independent Prescriber
Desirable criteria
  • Teaching qualification
  • Capability to work towards a professional doctorate

Experience

Essential criteria
  • Significant experience at band 8a or equivalent in relevant speciality
  • Significant experience of delivering education and training at post-graduate level
  • Portfolio of evidence showing achievements in practice/speciality
  • Broad knowledge on the use of medicines used within speciality
  • Evidence of presentations at local, regional, national level.
  • Evidence of leading guideline development and implementation
  • Evidence of involvement in service developments
  • Evidence of application of clinical governance skills to wider patient care
  • Publications and professional presentations.

  • Published in peer reviewed journals
