Job overview
Whilst this is a home-based post, you must have the ability to travel within Lincolnshire at short notice for training, development days, equipment updates and to provide shadowing opportunities.
This is a really exciting opportunity to join a developing service. The Here4You Access Team "went live" in January 2022, it was initially a 12-month project to but quickly established itself as a "must have" service.
Despite individual members of the team being based across the entire county of Lincolnshire we have become a very cohesive, supportive team.
This opportunity is due to staff development within our Emotional Wellbeing service line.
If you have a good level of understanding around Children and Young Peoples Mental Health difficulties and the services which support them and if you want to work in a fast paced, exciting and dynamic service with the needs of the children and young people of Lincolnshire at the heart of everything we do then this is the role for you.
Main duties of the job
The Here4You Access team practitioners' main duties are to screen all incoming routine referrals to Lincolnshire Partnership Foundation Trusts, Children and young people's emotional wellbeing and mental health services.
Taking calls on the Here4You line from young people, their parents/carers and fellow professionals. (Callers to our line are sometimes distressed and we can be required to take urgent action).
To provide advice and signposting information regarding other appropriate services available across the county and nationally.
Provide advice to professionals regarding emotional wellbeing and mental health of the CYPs they work with.
Liaising with other CYP services to ensure a positive journey for CYPs and their families when navigating the complex health and social care system.
Providing supervision and training to colleagues and students.
The practitioner hours are currently 9am until 5pm Monday to Friday, although occasionally this is extended due to safeguarding or risk management issues.

Detailed job description and main responsibilities
Provide clinical support to the team based on knowledge and experience as and when needed.
Supporting the induction of new staff.
Attendance at team meetings and away days where necessary.
Completing audits as required (for example quality audits and examining data).
Support the implementation of service improvements & changes within the team setting.
Act as Practice Supervisor or Assessor (registered nurse) for student nurses in the team.
Screening referrals to CYP services and liaising with peers to ensure the young person receives the right care at the right time.
Providing cover on the Here4You line to triage calls and provide support to professionals and parents/carers.
Completing thorough and holistic triage and risk assessments where necessary.
Liaise with other professionals/services re. referrals and according to CYP needs.
Recognition of own skills and professional capability to escalate cases as necessary.
Gathering feedback from children, young people, parents/carers and other professionals to enable service growth and development.
Proficient use of the clinical system to upload and locate demographic information as well as clinical information where appropriate.
Ensuring accurate and appropriate information is recorded within the young person’s clinical notes.
Robust risk assessing and triage including ensuring all information is accurate, timely and relevant to CYP care.
Person specification
Qualifications
Essential criteria
- RMN/ RNLD/Social Worker/Occupational Therapist with current registration. Or
- Educated to degree level in mental health/social care or another relevant subject or equivalent experience within the mental health field of working
Desirable criteria
- Therapeutic interventions qualification
Experience
Essential criteria
- Experience of working with people with mental health needs, including those with emotional wellbeing needs
- Experience in risk management of people with emotional wellbeing/mental health needs
- Good knowledge of universal children’s services
- Good knowledge of children and young people’s mental health and emotional wellbeing services
Desirable criteria
- Experience of working with children and young people who have mental health or emotional wellbeing needs
- Experience of delivering therapeutic interventions
- Local knowledge of children’s services
- Experience of providing support to junior staff
Special requirements
