Job overview

Lincolnshire Community Health Services are seeking a full time District Nurse to join them within the Lincoln North  Community Nursing Team. 

Applicants should hold the Community Specialist Practitioner qualification and have experience of managing a caseload of patients and leading a team of staff in delivering excellent patient care. There is an opportunity for this post to be a development role for a Case Manager on a fixed term basis of 1 year to allow for competencies to be obtained - this development will be a band 5 post whilst the competencies are attained. 

The service delivers care to patients in a variety of community settings across a  7 day service covering the hours of 8:00- 18:30 (shift work involved). 

Main duties of the job

The District Nurse/Case Manager will hold a caseload of patients with varying health needs, coordinating care delivery.  They will need to have extensive communication, interpersonal and clinical skills in order to be successful. 

The District Nurse/Case Manager is not only integral to leading on patient care but leading the community nursing team through challenging times and the transformation of services,  role modelling excellent behaviours which are in line with our trust values. The District Nurse/Case Manager is essential to improving the quality of care that we deliver and in ensuring that the patient has a positive experience.

Detailed job description and main responsibilities

As this is a community post, you must be able to travel to patients homes and clinic settings. Due to the rurality of Lincolnshire some areas are not well covered by public transport therefore having access to transport is essential.
