Job overview

The team provide a full range of healthcare to vulnerable children and young people (CYP) aged between 10 and 18 who are resident in the secure children home.

Resettlement can present a challenge for CYP leaving the secure estate, with many CYP expressing significant anxiety regarding the transition process. There are a variety of factors that may make the transition difficult for both the young person and the destination they are transitioning to.

You will support a CYP through this journey, by individual work, group work and liaison with families/carers. They will be integral to supporting constructive resettlement to promote change and prevent relapse.

You will encourage and promote positive leisure time and create individual personal support plans with young people. The principle of the child first and the offender second approach is used throughout the team alongside the trauma informed delivery of care due to the CYP having often experienced many adverse childhood events and trauma.

You will provide professional advice and support to other teams, agencies and members of the multi-disciplinary team and work flexibly between 9am and 9pm, including some weekends and bank holidays.

The team are committed to professional development and provide many training opportunities to allow staff to develop a wide range of skills

Main duties of the job

  • To offer children and young people (CYP) and their families/carers, within 150-mile radius a bespoke package of support starting at least three months prior to leaving LSU, but ideally within 6 months prior to leaving and for the first six months of a CYP’s community experience post custody.
  • To deliver care planning and individual and group interventions, in partnership with CYP and other workers involved in care delivery, under the supervision of the Band 7 post holder.
  • Direct work with Family – using the Signs of Safety Model (Lincolnshire is accredited in this approach).
  • Direct work with CYP within LSU preparing them for leaving custody with set targets.
  • To build up support networks available pre discharge through Local Authority, voluntary Sector, family, local activity centres and appropriate educational facilities.
  • To work in accordance with relevant childcare and health legislation and professional standards.
  • To proactively involve CYP and families/carers in service development and evaluation.
  • To function as part of the multidisciplinary LSU service.
  • To support the provision of a professional, non-stigmatising, trauma informed service to CYP and families/carers.
  • Work collaboratively, and promote effective working relationships, with members of the multi-disciplinary team, ensuring effective clinical decision-making, internally and with external age.

Person specification

Qualifications

Essential criteria
  • Level of education to NVQ 3/AS level or equivalent
  • Satisfactory level of Secondary Education to GCSE English A-C grade or 9-4 grade or equivalent.
  • Demonstrate a good standard of English and good verbal and non-verbal communication skills.
  • Willingness to undertake further training relevant to role.
Desirable criteria
  • Mental Health or Health and Social Care related qualification.
  • Recovery college course qualification

Experience

Essential criteria
  • Relevant experience of working with young people who have accessed children’s social care services and or Health and Justice services.
  • Experience of being in a role supporting or mentoring others
  • Experience of working in a team or group environment
Desirable criteria
  • Experience of working with young people who are Care experienced* or having contact with the youth offending and/or the criminal justice system.
  • Experience of working with children, young people, and families
  • Experience of working in a team
  • Experiencing of working with a range of different organisations.
