Job overview

We are seeking a medical consultant for our newly established Virtual Ward with an Acute medicine focus (1.5 PAs) combined with a Rheumatology specialty role (4.5 PAs).

 This is an exciting role providing clinical oversight to support management of patients on the Virtual Ward and further development of the Virtual Ward model in line with local and national strategy.

 A Virtual Ward is a safe and efficient alternative to NHS bedded care enabled by technology, supporting patients who would otherwise be in hospital to receive acute care, monitoring and treatment in their own home. The national ambition is that all systems will create capacity of 40-50 Virtual Ward beds per 100,000 population an ambition we are looking to achieve.

 In Barnet the Virtual Ward went live in early 2023 and provides specialist led care for a variety of patients including those with respiratory conditions, frailty and acute kidney injury. There is anticipated expansion into a number of other areas during 2024/25 and a plan to increase both capacity and occupancy. This care is provided to them in the comfort of their own homes using an advanced personal remote monitoring platform, and in reaching hospital at home nurses enabling them to safely leave hospital sooner.

Alongside Virtual Ward and Acute Medicine we are able to offer 4.5 PAs per week in our Rheumatology Speciality running mainly outpatient clinics and once weekly ward inpatient advice/cove whilst exploring service development opportunities.

Main duties of the job

The job plan, subject to review is as per the below:

8 PA – DCC

 

DCC will consist of:

·         Virtual Ward MDT

·         Hot Floor clinical cover

·         Patient ward round

·         ED in-reach

·         Post admission review

·         Review of referrals into VW

·         Clinical advice and support

Rheumatology Element:

·         Rheumatology clinics (4 per week)

·         HOT clinic (allied to AEC) alternate week.

·         Once weekly ward/inpatient cover.

2 SPA

SPA will consist of:

·         Service development and planning

o   Liaison with specialty leads, looking at potential service expansion

o   Presentation and updates to internal and external stakeholders

o   Participation in various meetings with internal and external stakeholders

·         Teaching and training of medical students and more junior doctors

·         Continuing professional development

·         Involvement in clinical governance (of both VW and other dept activities)

Essential criteria
  • Demonstrable ability to meet the Trust Values

Education & professional Qualifications

Essential criteria
  • MBBS or equivalent
  • On the specialist register of the General Medical Council (GMC) for the relevant specialty
  • Registered with the GMC and within 6 months (at the scheduled time of the advisory appointments committee) of being accepted for the specialist register
  • FCEM or equivalent
Desirable criteria
  • Postgraduate medical degree

Experience

Essential criteria
  • Clinical training and experience in the relevant specialty, equivalent to that required to gain entry to the UK Certificate of Completed Training (CCT) in Acute Medicine
  • Experience of and commitment to teaching undergraduate and postgraduate medical staff.
  • Experience of, and commitment to medical audit
  • Current ATLS,APLS or/and ALS
Desirable criteria
  • Instructor in ATLS, APLS or/and ALS
  • Special interest with extended experience in other specialty eg Endocrinology)

Skills and aptitudes

Essential criteria
  • Ability to take full and independent responsibility for clinical care of patients
  • Aptitude for teaching and training
  • Ability to cover on call.
  • Demonstrable commitment to multidisciplinary team working
  • Positive approach to lessons learnt
