An autonomous role, with a key part in supporting major trauma network & trauma patients. Responsible for coordinating day-to-day management of major or/and any trauma patients from admission to early rehabilitation phase. Responsible for liaising with members of multi-disciplinary groups of individuals,  from specialities of emergency medicine, intensive care, surgery, nursing, allied health, & support staff, who work as a team to assess & manage trauma patients. Will involve participating, lead training & education within ED & wards, chair & manage Trauma nursing/AHP steering group, liaising with  TARN data analyst & trauma lead.  Will be integral to ongoing clinical governance of major trauma unit, contributing to  day-to-day clinical implementation of service improvements. Responsible to work with TARN (trauma audit & research network), which allows networks, major trauma centres, trauma units, ambulance services & individual clinicians to bench mark their trauma service with  providers across the country.  Expected to be an effective communicator across MDT with strong leadership skills & ability to act as a senior professional role model. Key aspect is to enhance patient experience by acting as single point of contact between families/carers, medical team, therapies, & trauma related rehabilitation teams, to evaluate systems of care & plan service development & improvement. Will include attending trauma calls, ensuring best trauma practice is followed & scribing  calls.

 The Trust implements a Smoke Free policy that applies to all staff. Staff are not allowed to smoke while wearing a recognisable Trust uniform or visible trust identification badge, & not allowed to smoke anywhere on hospital grounds. Staff are  not allowed to take additional breaks in order to smoke. They may smoke during designated breaks but only out of uniform & off site. To be professionally legally accountable for all aspects of own work, including management of patients in your care by adhering to professional Codes of Conduct & standards of Practice. Work collaboratively across the hospital to coordinate care pathway of all trauma patients who require ongoing inpatient & community rehabilitation following a traumatic injury.  Undertake all aspects of clinical duties as an autonomous practitioner by facilitating & coordinating an effective pathway for patients.  Work as a key member & bleep holder of the trauma team under trauma call activations under the guidance & direction of the Trauma Lead to provide a coordinated service with excellent care of patient.  Work within Trust clinical guidelines & policies. Have a good working knowledge of national & local standards, monitoring own & others quality of practice with reference to these, e.g. vulnerable adult policy & child protection policy.  Ensure t the care provided & services delivered are in line with National Services Frameworks, NICE guidelines, Social Care Policies & Procedures   Clinical Governance Framework. Understand the legal framework around gaining informed consent for treatment & to implement appropriate procedures in line with trust policies.   Work closely with  ED trauma team from the time of patient arrival & be part of LAS hover, this will provide the post holder with a comprehensive understanding of patient’s clinical presentations & facilitate best practice to achieve TQUINS, BPT, CPG & national & local standards of trauma care.  To clarify rehabilitation referral pathways & establish new efficient working practices to facilitate timely, safe & appropriate discharges. To act as advocate for patients, both adult & paediatric, with trauma-related complex rehabilitation needs admitted to the acute hospital, coordinating their rehabilitation discharge from the trauma unit.  Work closely with teams across the network to ensure  all patients have robust plans for admission & treatment to maximise best clinical outcomes. Will require sensitive negotiation skills with all members of the MDT.   Provide highly specialised advice to patients, family, carers, health & social care professionals regarding the management of patients with trauma related injuries.  Empower patients to make informed choices regarding their rehabilitation & reintegration & to advise patients on the availability & limitations of community  resources.  Use specialist knowledge to ensure accuracy of rehab prescriptions that evaluate & direct the patient’s social, psychological & physical requirements.  Display highly developed written & oral communication skills for effective liaison with the MDT & patient/ family/carers re: status of rehabilitation referrals & requirements for acceptance/transfer.  To support Bed & Site team with the repatriation & transfer of trauma patients to the correct clinical specialty, ward location or trauma centre.    To support clinical teams in decision making around a transfer & repatriation of trauma patients.   Work closely with  AAU team & frailty team to develop & improve silver trauma pathways & coordination.  Attend board rounds relevant to trauma patients within the hospital to influence timely patient transfer & coordination of care.   Attend  ward rounds ensure all action plans from ward rounds are carried out in a timely way.

 To coordinate & facilitate effective pathways of care for trauma patients optimising their safety wellbeing.    Ensure all major trauma admissions are highlighted to the service. When overnight or at the weekend, this should occur at service resumption the next working day.   To maintain an excellent knowledge base that reflects contemporary issues in trauma care.    Have an overview of all major trauma patients in the organisation at any one time regardless of which speciality they are under.     Attend relevant ward rounds ensure that all action plans from the ward round are carried out in a timely manner.   Provide specialist advice peer support to AHP, nursing, medical, social care educational colleagues in the overall management of the trauma patient presentation.   Work in partnership with the trauma therapists, discharge co-ordinators bed management team to ensure swift clinically effective repatriation discharge of trauma patients.     To ensure that each individual has a tailored contemporary rehabilitation prescription care plan developed through a collaborative process to reflect their individual needs rehabilitation goals.    Where appropriate lead on, participate in key developments relating to trauma care so that high quality, patient focussed services are promoted.    Attend participate in multi-professional team meetings.     Have an active role in trauma governance, in conjunction with the trauma lead TARN data analyst.   Participate collaborate in peer review, working closely with  trauma lead. Submission of data collation of trust data to support Trauma Peer Review annually   & CQC.   Set, monitor review standards of care on  regular basis, initiate corrective    action plans.    Develop effective processes in conjunction with  TARN data analyst to identify track trauma patients  through the hospital, be aware of Datix clinical incidents morbidity mortality rates associated with trauma in order to make recommendations for improvements to the trauma pathway based on this data.  Facilitate a high quality, cost effective, specialist service through monitoring, audit, implementation of KPIUs, patient organisational outcome standards, including BPT, TQUINS national local standards  of trauma care.    Work as a key senior member of the trauma team under the direction guidance of trauma lead to provide a coordinated service with excellent care for patients.  Manage, oversee maintain vital equipment required to run  trauma  service.    Implement new ideas initiatives where possible to run a cost effective timely service.     Progress own CPD by keeping up to date with key trends developments  by reading attending external/ internal courses in-service training sessions.   To adhere to the principles of lifelong learning maintain CPD  portfolio reflecting personal professional development.     To participate in  staff appraisal scheme personal development planning (PDP) as both appraiser appraisee to assist all staff in setting objectives in line with planned service developments personal career objectives.    To be responsible for managing own time effectively.     Demonstrate safe use of equipment efficient use of resources.    To ensure the best use of available resources is used within agreed budget to provide a cost-effective service.  To apply sound financial evaluation to proposed service developments.  Act as a facilitator, mentor, preceptor teacher to all relevant staff groups offering guidance, support education (both formally informally).     Assess own educational needs take steps to keep up to date with clinical developments.  Work closely with the Matron, Sister/Charge Nurse Practice Development Teams clinical lead therapists within the ED wards, ensuring that relevant clinical based teaching programmes are in place for all staff.     Teach on relevant courses training programmes within the trauma service develop training packages as required, mapped to the national adult paediatric trauma competencies for all staff groups. specifically to support advise CPEs in level 2 trauma competencies for ED staff level 1 trauma competencies for wards receiving trauma patients.     Coordinate the development delivery of a patient focussed education information giving strategy within the speciality.     Motivate, develop, support identify training needs within the trust around the care of trauma patients.    Ensure Network offered education opportunities are utilised by relevant staff groups.  Provide a leadership role model demonstrate expert  knowledge high standards of clinical practice.    To contribute, review participate in Trauma Governance activities – Datix, SIRP, Serious Incident review work with the MDT processes to effectuate closure outcomes.

Maintain accurate records of the clinical service ensure that confidentiality of  information is adhered to.     To ensure compliance with the Data Protection Act RFH Trust policies in management of information concerning patients, clients staff.  Promote undertake specialist research, updating on knowledge to promote excellence in clinical practice.  Participate in be aware of clinical research trials for patients.   Contribute to audit research within the trauma speciality in collaboration with colleagues, developing action plans disseminating information.   To be familiar with current clinical research protocol issues relevant treatment procedures employed in the diagnosis treatment within the speciality of trauma.     Work with TARN Data analyst offering clinical guidance, clinical advice support on the data collection uploading process, cross covering the TARN data analyst for annual leave times of absence to maintain Trust TARN data trajectories.    Ensure TARN data accreditation ascertainment targets are adhered to strive to meet set Network goals.     Participate in the promotion of the speciality by publishing presenting innovations research findings locally nationally.    To be responsible for maintaining accurate comprehensive patient treatment records in line with legal requirements trust guidelines.  To work with the trauma lead to contribute to ongoing development of clinical practice, training, research, standards of care within the service including policies, protocols guidelines.  To undertake clinical audits research within specialist area   To undertake audits Qi initiatives relevant to Trauma Care within the trust.
