We are seeking an enthusiastic person with excellent communication skills to support intermediate care interventions alongside our friendly multidisciplinary team on our  ward at Mountnessing Court.

Main duties of the job

The applicant needs to be someone who understands the uniqueness of older people and is able to work within an inter-professional team to deliver a rehabilitative service to meet the patient’s needs.  Previous experience as a health care assistant in an inpatient setting is required for this role.

Intermediate Care interventions are designed to prevent the need for acute services for older people and the development of a new model of care for patients providing holistic assessment and rehabilitation aims to achieve this.

If you think you have the drive and experience and want to make a difference, we look forward to hearing from you.
