Senior Sister - Continuing Care Team

Are you looking for a challenge or a change? Want to use the wide range of skills you have built up over your years working as a qualified Children's Nurse? Are you looking to support and care for some of the most complex healthcare needs children in our borough, who are living in the community?
If so then we want to meet you. We have a 12 Month fixed term contract at 22.5 hrs per week in our Continuing Care Team, where children are assessed for packages of care that meet their specialised needs in the community, keeping them well and out of hospital.
You would be working alongside experienced members of the team, to support and guide you along the way. What do we need from you? A wide range of nursing experience caring for children with long term, complex health care needs, ITU/Long Term Ventilation experience a bonus.

The post holder will play a key role in managing the Children’s Continuing Care caseload within the Community Children’s Nursing Service in Newham, working within the remit of the current Children and Young People’s Continuing Care National Framework. This includes delivering an effective and person-centred service for children, young people and their families with the highest level of medical needs in the borough, including those that are technology dependent, highly complex and/or requiring palliative care.
The post holder will play a key role in leading and developing the Continuing Care Team, working in conjunction with the Children Community Nursing Team (CCNT), Diana Palliative Care Team and external stakeholders, to implement effective service pathways, ensuring the delivery of key quality outcomes.
An essential element of the role will include seamless working with other professionals and partner agencies when assessing eligibility for Continuing Care and supporting multidisciplinary teams in the assessment process. The role will ensure that the provision of care achieves individual outcomes and ensures the delivery of efficient quality packages of care that is aligned to the Personalisation and SEND agenda and supports the transition of young people into the appropriate adult services.

- Experience of managing staff and resources.
- Experience managing complex discharges from hospital to community
- Experience of inter-agency working.
- Experience of managing complaints and incidents.
- Experience of performance and sickness management of staff.

- Experienced practitioner in long term ventilation and tracheostomy care
- An expert in assessing the holistic needs of children with complex needs designing subsequent packages of care.
- Excellent knowledge of the NHS framework for continuing care.
- An advocate for children, young people and their families.
- Excellent written and verbal communication skills including providing and receiving sensitive or contentious information.
- Excellent teaching skills with an ability to adapt these for use with health care support workers, parents, children and young people.
- Able to represent health at integrated service meetings presenting findings of continuing care assessments.
- Knowledge and understanding of child health needs across a culturally diverse population.
- Act as a specialist resource for children, young people, their families and inter agency staff in planning end of life care.
- Compassion and sensitivity in engaging and building supportive working relationships with children and families experiencing distress and major life transitions
- Willingness to work on call rota to support end of life care
