Job overview

The Recovery and Rehabilitation team provides a Monday to Friday, 9-5 service that supports clients with Complex mental health needs, who are in Croydon funded residential and supported living placements. Many of our service users have multiple diagnoses including substance misuse, mild learning disability or ASD, physical health or disability issues as well as severe and enduring mental illness, often treatment resistant psychotic disorders, and secondary personality disorders. 

We work in a collaborative way, to enable recovery and manage mental health symptoms, whilst promoting the development of activities of daily living skills, working towards step down to more independent accommodation and less restrictive options. We are an integrated SLAM and Local Authority Service, dedicated to supporting our service users to achieve their optimal independence and choice through skills building, illness management, and assets-based, person centred care.

 

Main duties of the job

The care coordinator's role within the team is to support clients with practical and therapeutic interventions to drive their rehabilitation plans forward, and aid their recovery to maximise their ability to step down into lower supported accommodation.

As part of your professional development you will be expected to lead and supervise junior staff and students, and will be encouraged to take part in your own clinical supervision and appraisal.

Within this role, we will expect you to complete holistic assessments, including mental state, physical health, and risk, whilst creating collaborative person-centred care plans using SMART goals with service users.

About the Location

Queens Resource Centre, 66A Queens Road, Croydon, CR0 2PR. The Nearest Train Stations are West Croydon  (0.9 miles away) and Thornton Heath (0.8 Miles away). There are several buses that run locally, some stop just outside the centre, others are a few minutes walk away. There is a small car park on site available on a first come first served basis.

Detailed job description and main responsibilities

Clinical Skills

Working autonomously within the parameters of the role leading and supporting junior staff and wider care team to implement the delivery of quality nursing care.

Knowledge and Frameworks

Demonstrating an enhanced understanding of Mental Health signs and
symptoms and an understanding of how these may present in practice including recognition of impact on patient presentation and behaviour including risk factors.

 

Person specification

Qualification

Essential criteria
  • Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse/Registered Occupational Therapist
  • Mentorship/Practice Supervisor and/or assessor Qualification
Desirable criteria
  • Relevant Master’s degree
  • specialist training in interventions for people with Psychosis and/or ASD

Experience and Knowledge

Essential criteria
  • Evidence of reasearch based/reflective practice and ability to use supervision to refelct on skills, attitude and knowledge
  • Significant experience in rehabilitation and recovery.work with service users
  • Experience as a preceptor or mentor
  • Knowledge of clinical governance and audit
  • understanding of Care coordination under CPA
  • Evidence of understanding all relevant Health and social care legislation (MHA, MCA & DoLs; Health and Social Care Act etc.)
Desirable criteria
  • Experience of managing a caseload
  • Experience of supporting practice development in clinical areas
  • Awareness/knowledge of the issues surrounding the social inclusion of mental health service users

Skills

Essential criteria
  • Evidence of Assets-Based, Person-Centred, collaborative assessment and care planning skills
  • Evidence of ability to prioritise work and meet KPI targets
  • Excellent communication skills including written and oral skills, listening skills, negotiating skills and use of technology to support communication.
  • For NURSING POSTS Evidence of nursing competences, knowledge and skills (e.g physical health screening, assessing mental state, etc)
  • Phlebotomy trained (Nurses) or agreement to achieve this as a priority.
Desirable criteria
  • Evidence of skills in specific skilled interventions [e.g therapeutic interventions (CBT), Specialist assessment (MOHOST)
  • Evidence of leading safeguarding investigations
  • Evidence of Developing Behavioural Support Plans.
