About the team
This is an exciting development in Mental Health Service provision in Lewisham and an opportunity to work across the borough promoting the group programme and occupationally focussed interventions. The service aims to provide proactive, strength-based care for service users experiencing mental health problems in a primary care setting using a stepped care model.
The primary Care Mental Health Teams provide care and support to service users who require a level of care above that available through general medical services, but do not require secondary care mental health intervention.
The team also support service users who have on-going, low support needs and those who are transitioning from secondary care mental health services to primary care services.
The Primary Mental Health Team incorporates roles in social prescribing and care navigation, drawing on the expertise of third sector partner providers. In addition to occupational therapists, the team comprises mental health advisors, peers support workers, nurses, social workers, consultant psychiatrist and will be co located with MIND and IAPT psychological services and the service user network SUN.
