Palliative Care Clinical Nurse Specialist

The North Middlesex University Hospital NHS Trust is a medium-sized acute and community trust with over 525 beds and over 600 community staff, serving more than 600,000 people living across Enfield and Haringey and the surrounding areas, including Barnet and Waltham Forest.

The hospital has been on its present site for over 100 years and was established as an NHS trust by statute in December 1990. The Trust’s services are provided on the North Middlesex University Hospital site as well as a range of community sites across the London Borough of Enfield, including at partner hospitals. They provide services in collaboration with a range of partners, including local GPs, acute, mental health and other community health service providers across North Central London.

In the year ending 31 March 2022, the Trust reported a turnover of £419.7m and employed almost 4,000 staff. Following the transfer of Enfield Community Services on 1st April 2023, this has increased as we have welcomed over 600 new staff including District Nurses, Community Matrons, Community Physiotherapists, Psychologists and many more across a wide range of adult and children’s community services in Enfield. Job overview

We have an exciting opportunity for 2 x Band 7 CNS to work within our highly skilled, motivated and dynamic team. We aim to make a difference and improve the experience of palliative care patients at various stages of their disease trajectory.

The post will be based within the existing inpatient Palliative Care Team. We offer 7 day face to face support to ensure that people with specialist palliative care needs, will receive the best quality care.  The team currently consists of 4 palliative consultants’ equivalent to 2.5 WTE, 1 Band 8a Lead Nurse, 4 WTE Band 7 CNS's 1 WTE band 6 Associate CNS 1 WTE Band 7 palliative care fast track coordinator.

The post holder will act as a specialist resource Trust wide. Using Specialist Palliative skills and knowledge you will manage a caseload of patients and support their significant others and carers through effective, efficient evidence based high quality care.

The delivery of education and training to multi-professional groups within the Trust is an integral part of the role.

The post holder will also contribute to the teams’ development, research and governance through the process of audit and bench marking.

We welcome applicants with existing palliative care experience or equivalent.

Main duties of the job

·         To effectively assess, plan and provide specialist care and support to people affected by life limiting illnesses, and also to patients who are at the end of their lives.

·         To participate as part of a multi-disciplinary, working collaboratively with colleagues, patients, carers, other health and social care professionals, statutory, voluntary and independent organisations to ensure optimum patient management

·         To work within statutory legislation and local policies and procedures in relation to discharge planning and palliative care.

·         To work autonomously and manage own case load

·         To participate in providing a 7 day face to face service

·         To act as a resource, educator and advocate for patients, their carers and health care professionals.

·         To participate in education initiatives to all members of staff

·         To participate in audit/research.

Detailed job description and main responsibilities

1.1 Acts as a resource of clinical excellence in the speciality of palliative care nursing, and ensure that national and local policies are implemented and followed

1.2 Assess, plan, implement and evaluate holistic patient care based on sound rationale and evidence-based practice to influence and implement treatment

1.3 Participates in the management and triaging of new referrals and taking part in the daily allocations

1.4 Maintain and be responsible for electronic and written records in line with the organisation

1.5 Offer skilled emotional and psychosocial support and refer on to other members of the multidisciplinary team as appropriate.

1.6 Support the Continuing Care Application process and complex discharges where appropriate.

2.1 Act as a patient advocate

2.2 In collaboration with other services and the multidisciplinary teams deliver a high standard of care for the benefit of patients and the public

3.1 Ensures the Palliative care policies are updated in response to national and local changes to practice

3.2 Works collaboratively with external providers and agencies to align with regional and national initiatives 

5.1 Managing the caseload in an effective, efficient and timely manner

Education & Professional Qualifications

Essential criteria
  • • Degree in Nursing
  • • Registered Nurse (RNA)
Desirable criteria
  • • Teaching qualification
  • • Post registration course in specialty
  • • Masters in related subject

Experience

Essential criteria
  • • Palliative care experience in relevant field
  • • Knowledge/understanding of confidentiality issues
  • • Working in the healthcare sector with relevant oncology /Palliative care or other relevant experience
  • • Understanding of the palliative patient, family and carers issues and their role in service development
  • • Experience in working across a variety of settings
Desirable criteria
  • • Teaching/educating nurses, AHP’s and/or patients
