The Integrated Crisis Assessment Hub is seeking to recruit 3 full time Band 5 Registered Mental Health Nurses with a minimum of 6 months experience working in inpatient setting.

Applicants are expected to have a clear understanding of the Integrated Crisis Assessment Hub/Crisis Resolution model in the delivery of care to our client group.  Applicants should have the ability to work in a highly pressured environment, work as part of a multidisciplinary team and able to make autonomous decision.

Practitioner has the responsibility for all service users who present to the Hub as walk in’s, diverted from ED or through the triage process. Their role is to ensure that service users within the Hub are supported at all times. This role is a Band 5 and will have a clinical input

  1. The ICAH Practitioner will work very closely with the ICAH Health Care Support Worker to ensure that client’s physical health state is monitored on arrival e.g. vital signs on arrival, COVID monitoring
  2. Their role will involve monitoring the safety of service users within the Hub. They will have an overall responsibility for the waiting area.
  3. The role will entail supporting Senior Practitioners during an assessment of service users in the clinical area or assessment room.
  4. The Senior Practitioners will ensure that the triage tool is completed when service users arrive at the Hub.
  5. The role will assist the ICAH Clinical lead to complete service user’s feedback and also be responsible for completing the Hub Admission logbook to maintain record keeping of outcomes.
  6. The role will also assist in dealing with telephone triaging which involves taking referrals and dealing with queries relating to patient care

Probationary Period 

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High-Cost Area Supplement

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436) (pro rata for part time staff).

Experience

  • Working with people with complex, acute mental health needs
  • Undertaking audits within a clinical setting.
  • Assessing, planning, implementing and evaluating aspects of care.
  • To have experience in working / liaising with multi-disciplinary teams and other agencies in the provision of care.
  • Ability performs under pressure, quickly and effectively, making timely managerial decisions in conjunction with the Team Manager and following through available agreed care pathways.
  • Ability to work independently and as part of the team

  • Experience in working in Mental Health Liaison Service

Knowledge

  • A clear understanding of the Crisis Resolution Model of service delivery including gate keeping and facilitate early discharges from the ward.
  • The 1983 Mental Health Act and related legislation, codes of practice and guidance
  • Safeguarding Children and adults.
  • Excellent working knowledge of intensive care programme approach.
  • Medication management
  • Physical Health assessment.

Skills

  • Well-developed interpersonal skills especially when handling conflict and change.
  • Ability to facilitate positive multi-disciplinary working relationships across all services areas.
  • Ability to explore emotionally demanding aspects of the role with appropriate staff in both managerial and clinical supervision as well as in team meetings and informal work related support systems
  • Ability to take positive risk and provide care to service Users, in the least restrictive environment.
  • Ability to maintain confidentiality where appropriate.
  • Understanding of and commitment to the principle of equal opportunities in service delivery.
