Hammersmith & Fulham a great place to grow old.   The Community Independence Service (CIS) has an opportunity for a Band 6 Occupational Therapist to join our rehabilitation team.

The Community Independence Service (CIS) provides planned and unplanned nursing and therapy interventions for older residents who have had a recent deterioration in their physical health and is run jointly between West London NHS Trust and CNWL Foundation Trust across the three inner London Boroughs.

We are achieving excellent results in community rehabilitation and admission avoidance for the people of Hammersmith and Fulham and are part of a wider community independence service offered in Westminster and the Royal Borough of Kensington & Chelsea. 

Main duties of the job

As a Band 6 Occupational Therapist you will provide community based assessments alongside intervention planning for individuals deemed appropriate for the rehabilitation pathway. Using a holistic approach, you will provide an individualised intervention plan for service users, including but not limited to equipment provision, home exercise programming, activity planning, functional skills practice, carer involvement, environmental recommendations, advocacy work, motivation and confidence building and education and advice.

You will be working closely with our acute partners, such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.

We will connect you with peers across the tri-borough of Hammersmith and Fulham, Westminster and Kensington and Chelsea to support you and to work on service improvement partnerships.

We will ensure you are able to follow the clinical and leadership development priorities identified in your personal development and appraisal plan.

We will equip you for mobile working and actively support flexible working practice when required and appropriate for the service.

Qualifications

Essential criteria
  • Degree / Diploma Occupational therapist
  • Registration with HCPC as an Occupational Therapist
Desirable criteria
  • Member of RCOT and or involvement in professional network
  • Post Graduate MSc Degree
  • Post-registration courses

Experience

Essential criteria
  • Previous experience of working in the UK or a similar health economy (acute or community) as an occupational therapist.
  • Previous general experience
  • Experience teaching juniors and assistants
Desirable criteria
  • Experience working as an autonomous practitioner in a community setting
  • Previous general experience
  • Experience teaching students

Knowledge

Essential criteria
  • Excellent communication skills and fluency in written and oral communication.
  • Ability to maintain accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the Trust policies.
  • Ability to talk to people about sensitive issues in a supportive manner but also challenge others assertively where necessary.

Skills

Essential criteria
  • Understanding and ability to apply relevant knowledge and skills required to act as an effective professional clinician and adhere to your relevant code of conduct/practice.
  • Ability to plan and use assessment methods that are appropriate for complex needs, and use processes of reasoning that are based on available evidence including national guidelines such as NICE and other local and professional policies.
  • Ability to follow processes of reasoning which balance additional information against the overall picture of the individual’s needs to confirm or deny developing hypotheses.
  • Knowledge and understanding of the role of intermediate care in supporting a timely discharge.
  • Knowledge and ability to use Microsoft Office suite for various administration tasks
  • Working with spreadsheets, document writing and email
