Job overview

BHR Falls Prevention Service is looking for an enthusiastic and skilled Nurse  with Falls Experience or an interest in Falls, to lead the nursing team and to deliver falls prevention in the community.

We are seeking the right nurse who can

  • Provide a high standard of nursing care to Falls patients in their own home or in a group setting
  • Support the multi professional team in implementing Falls Prevention Programmes 
  • Work effectively in a team environment

Main duties of the job

  • To lead BHR Falls nursing team 
  • The sucessful candidate will be supporting the Falls Care Home Pathway
  • Identify and assess patients who have been identified as having risk of falls by completing a Community Falls Assessment 
  • Ensure up to date knowledge and skills, advise and share knowledge within the team and the wider community
  • To participate in the development of an effective team and the development of productive working relationships, including supervising staff  
  • To monitor the patient’s progress/status throughout their care pathway and liaise with the patients GP, medical staff or other community services as appropriate
