CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
An exciting opportunity has arisen for a B6 Deputy Team Lead to join our Community Adult Planned Care team based in Harpenden Memorial Hospital. The team consists of Community Nurses, District Nurses, Community Paramedics, Physiotherapists, Occupational Therapists, and Health Care Assistants.
The Community Nursing team covers 8am-8pm, 7 days a week. Our aim is to be the best we can, offering safe, patient-centred and evidence based care to those accessing our service. As a Deputy Team Lead you will lead a small team of staff and will be responsible for managing a caseload of patients, ensuring that a high standard of care is delivered.
You will have regular input from the Professional Development Nurses and your Team Leader to support you in gaining your competencies on the Community Nursing Pathway. As a service we offer a range of different opportunities to develop your skills. Once you have completed the Community Nursing Pathway we can support you to apply for the Deputy Team Leader pathway.
Main duties of the job
To set and maintain standards of care and deploy, co-ordinate and supervise the nursing team and learners as required.
To provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.
To be responsible for the team’s assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8pm-8am
To be responsible for the team’s maintenance of patient and other records
To participate in the long term management of patients within the locality
To have continued responsibility for a defined caseload.

Essential criteria
- Registered Nurse
- Practice Supervisor/Assessor or equivalent
- Specialist Nursing Practice(District Nursing) Degree or will undertake training
- Nurse Prescribing or will undertake training
- Knowledge of recent developments in NHS
- Knowledge of Evidence based practice
- Evidence of personal development
- Knowledge of NMC code of professional conduct
Desirable criteria
- First Level Degree
- Independent Prescriber
- Relevant courses in Long Term Conditions Management
- Course of Top to Toe examination/ diagnosis Experience
