An exciting opportunity has arisen to work as a Team Leader  for Harrow Community Mental Health Triage and Assessment Team. We are looking for an experienced Mental Health Nurse to join the team to work in the community with Adults with mental health needs and support the team with the day to day running of the Triage Team.

The role is suitable for nurses who have experience in working in the community and for someone seeking to develop their leadership and management skills

Harrow Community Mental Health Triage and Assessment Team, based in Harrow and Wealdstone is looking for a passionate RMN who is looking to make a difference whilst working in an environment that promotes staff development and training opportunities. CNWL is passionate about developing its own staff, driving them to go on to become more established practitioners and this post is no different.

The CMHH is an exciting area to work in as it is at the forefront of mental health transformation in line with the five year forward plan.  To be considered for this post you will need to be a registered mental health nurse and be keen to develop experience of working in multi-disciplinary teams who care for adults within mental health secondary care sector. 

Main duties of the job

  • The post holder will work in partnership with other teams within the borough and provider agencies to refer service users who need long term support as necessary.
  •  To provide brief clinical and social care intervention for service users.  
    •    Screen and triage referrals and co-ordinate duty assessments.
  • Attend regular management and senior team meetings as appropriate and to deputise as necessary.
  • Ensure staff keep accurate electronic notes and input data as required.
  • When required, to undertake holistic assessment of the mental health needs of adults in the community
  • Draw up the monthly rotas
  • Develop and maintain close professional relationships with GP’s and other professionals.
  • Ensure staff practices in a safe, appropriate and professional way with service users and their carers.
  • To promote and support self-care, recognising the important role of individuals, their families and carers as co-producers of health.
  • To identify vulnerable adults and provide effective programmes of support that protect and promote their health and wellbeing, intervening appropriately when a person’s safety or welfare is at risk.
  • To assist the team to be compliant with KPI and monitor electronic data
  • To cover for the absences of senior colleagues as appropriate and as required by service needs.
  • To contribute to the implementation of the Clinical Governance agenda including Target Setting for Teams, Clinical Audit.

Detailed job description and main responsibilities

  • The post holder will work as a Clinical Team Leader in the Borough team providing brief clinical and social care intervention for service users. 
  • To Triage referrals and co-ordinate duty assessments,
  • Be responsible for the supervision of staff  & their case load.
  • Facilitate appropriate clinical practice of the team including CPN’s, OT’s, Administrative staff 
  • Draw up the monthly rotas
  • Develop and maintain close professional relationships with GP’s and other professionals.
  • Ensure staff practices in a safe, appropriate and professional way with service users and their carers.
  • Attend professional practice forums
  • Chair clinical and team meetings as appropriate.
  • Provide senior support to the Team Manager
  • To assist the team to be compliant with KPI and monitor electronic data.

Qualifications

Essential criteria
  • Relevant professional Qualification - RMN
Desirable criteria
  • Relevant post-basic training for all Disciplines
  • Mentorship qualification relevant to professions
  • Practice Educator Programme
  • Management training (All)

Experience

Essential criteria
  • Significant years post qualifying experience at Band 6 level
  • Working with people with severe and enduring mental health problems in the community.
  • Assessing health and social needs and formulating care-plans to reflect those needs.
  • Working in a multi-cultural environment.
  • Practical experience of providing high quality care and exceeding expectations.
  • Demonstrate an understanding of the Urgent Access Pathway (UAP) and threshold of Secondary Mental Health Services
  • Experience of caseload management and throughput
  • Sound understanding of the local care pathway and interface with other local teams
  • Experience of working in a multidisciplinary team.
  • Experience of providing supervision

Knowledge/ Understanding

  • Mental Health Act 1983, Care Act 2014, Mental Capacity Act 2015
  • Ability to embrace and manage change effectively & sensitively within the team and the wider organisation.
  • Knowledge of new developments in social care and allied health professions

  • Highly skilled in IT applications i.e excel, word
  • Proficient in utilising Tableau to monitor team/individual performance
