We have an exciting opportunity for an experienced ACP/ANP to join the Rapid Diagnostic service, this role is available either full time or as 30 hours/0.8WTE per week.  The post holder will provide direct care, advice, management, leadership, and supervision of high-quality patient focused services for patients referred into the rapid diagnostic clinic. They will be an advance autonomous practitioner working both independently and alongside other health care professionals to assess, investigate, diagnose and treat patients.

You will be a key member of the RDC multidisciplinary team. They will be the main point of liaison between patient and local GPs.

Prif ddyletswyddau'r swydd

The post holder will undertake triage of the patient after referral from Primary Care. They will also supervise and co-ordinate the patient’s investigative pathway between hospital departments. Together with the pathway co-ordinator and GP Lead, they will act as the point of contact between the patient and the various specialty teams involved in the diagnostic process. They will facilitate investigations prior to a clinical assessment wherever feasible and appropriate to ensure a timely diagnosis.
