West Middlesex University Hospital is a modern, busy acute hospital in West London. We provide a full range of services to local residents including accident & emergency, acute medicine, surgery, paediatric and neonatal services, oncology, haematology & ICU plus an award-winning maternity service. The Trust is also part of Chelsea and Westminster NHS Foundation Trust.
We are looking for an enthusiastic, driven and motivated pharmacist to join our expanding and patient focused specialist pharmacist team.
The post holder will be responsible for the development and delivery of clinical pharmacy services to our Acute Medical Unit. The pharmacist will also develop and implement policies, procedures and guidelines in order to drive service improvement and enhance patient care.
The post holder will work closely with the multidisciplinary team on the wards, and be involved in a diverse range of patients that ensures they receive the best care. In addition to this, you will be supported by a medicine management technician (MMT). You will also be in charge of the screening of prescriptions, working with the relevant teams to improve prescribing and participate in ward rounds. Working as part of a clinical pharmacy team, you will be fully supported to develop your career. You will be required to work weekends and shifts on a rotational basis.

- To provide a specialist clinical pharmacy service to patients within the Medicine Directorate
- To ensure the safe, effective, legal and accurate use of medicines within the speciality and to support and
supervise less experienced pharmacists on rotation.

Detailed job description and main responsibilities
Key areas of responsibility are:
- Be responsible and accountable for the clinical pharmacy service provided to the Acute Medical Unit and provide support to the pharmacy discharge team.
- Perform audits of drug usage within the Directorate as part of the multi-disciplinary audit process, based on national and service priorities e.g. NICE, CLAHRC research and locally agreed requirements.
- Participate in the training and supervision of junior pharmacists and pre-registration pharmacists in providing a clinical pharmacy service to medical patients
- Develop clinical pharmacy services in line with local and national objectives and contribute to pharmacy research initiatives being undertaken within the department

Essential criteria
- Degree in Pharmacy
- Registered with the GPhC
- Postgraduate certificate in Clinical Pharmacy or one year hospital experience
Desirable criteria
- Postgraduate Diploma in Clinical Pharmacy Practice
