We are looking for an enthusiastic RNLD or RMN to provide evidenced based care to our CAMHS LD service users at Crystal House who also have mental health problems and/or challenging needs. Our team that is committed to working at the highest standards of clinical practice and who seek to continually improve on this through engaging in regular Quality Improvement projects.

Main duties of the job

Working as a member of our LD-dedicated multidisciplinary team you will contribute to the care planning and risk management of our service users who may be detained under the Mental Health Act and work intensively with them and their carers, relatives and day service providers.

The service also takes part in CNWL’s highly successful Rotation and Development (RAD) Programme, allowing Nurses to rotate in 3 different LD services across both inpatient and community services over an 18-month period. We really are committed to staff development!

Detailed job description and main responsibilities

  • To work collaboratively with Managers and Colleagues on the unit to provide the highest standard of nursing and care to the young people. 
  • To plan, assess, implement and evaluate individualised care plan and risk assessment in line with the young people care and treatment plan and PBS framework. 
  • To ensure care provided to the young people are in line with Care Quality Commission (CQC)
  • Ensure effective communication, including robust handovers, MDT communication and liaison with families and other professionals.
  • To communicate complex and sensitive information effectively to young people, care staffs  and other members of the multidisciplinary team, overcoming any barriers to understanding and providing support during distressing or emotional events.
  • Devise support  care and treatment plans ensuring individuals capacity and health needs are considered, documented and incorporated. 
  • To develop and maintain clinical skills and knowledge necessary to provide holistic evidence based  nursing care. 
  • TO be competent in the administration and management of medication of medicine in line with CNWL policies and procedures.
  • To work and uphold CNWL trust values and demonstrate a professional approach to work and act in accordance with NMC Code of Professional Conduct at all times. 
  • Maintain confidentiality surrounding the service at all times. 
  • To participate in staff appraisal, staff development  and in service training  in line with the trust Mandatory and statutory training. 
  • To facilitate clinical supervision to junior staffs as a qualified nurse.

Education and Qualifications

Essential criteria
  • RNLD/RMN/ Registered with NMC
Desirable criteria
  • Successful in the completion of BCS nursing degree or Master’s degree in Nursing.

Experience

Essential criteria
  • Past Experience in Working with young people/Adults with learning Disability and Autism. (Optional)
  • Experience as a student Nurse undertaking BSC or MSC Nursing degree.
  • Experience in care planning.
  • Experience of assessing and managing clinical risk
