As a qualified Physiotherapist: Community Deconditioning Champion you will be managing the Hospital's relationships, referrals pathways and service delivery with community partners - a GP practice, a Nursing Home and the Hospital's Pre-Operative Assessment service to deliver the Falls Prevention: Community Exercise Programme and manage a small team of trained volunteers.  In addition to managing your own caseload of patients' homes and community spaces, you will also be in charge of supervising Physiotherapy Undergraduate Students on placement, collecting data to support our End PJ Paralysis Campaign.

You will spend some of your time on the Hospital's inpatient wards, supporting the Inpatient Physiotherapy Team and a small team of Physiotherapy Students to encourage patients to mobilise, guiding Gentle Movement (Exercise) Volunteers  and conducting physical outcomes assessments on wards in order to identify patients who are suitable for the home based, Falls Prevention: Community Exercise Programme. 

You will establish clear systems to ensure swift receipt of community referrals and allocation of a Trust volunteer to deliver the programme at home or in the community, managing all data safely and securely. 

You will support the management of volunteers who deliver the exercise programme in people's homes, a Nursing Home and our Pre-Operative Assessment unit, managing their time, supervising their case load, providing training and clinical guidance.

You will also have your own clients, visiting them in their homes, Nursing Homes and local warm spaces to reduce fear of falling and increase the physical strength, balance and coordination of elderly people across our local communities.

The inpatient physiotherapy team delivers a flexible and evidence-based physiotherapy and occupational therapy service to fit the different needs of our patients. For that service to be driven by a committed, compassionate and happy team, that puts life-long health and well-being of patient’s, carers and staff at the heart of what we do. Supporting our community to feel better and live well.

Clinical

  1. Clinical assessment & triage of patients to determine eligibility to participate in Falls Prevention: Community Exercise programmes and classes
  2. Taking escalation requests from volunteers and partner staff, e.g. GPs, Nurse and clinically advising on safety, safeguarding and ongoing action to protect the safety and wellbeing of patients and volunteers
  3. Running pre-designed group exercise classes, supported by volunteers in a range of clinical settings – GPs surgeries, community halls, Nursing Homes
  4. Training and advising clinical staff in partner organisations to promote the Re Conditioning Games and advise on ways to prevent elderly service users from de-conditioning in the community and nursing home sector
  5. Provide 121 and group clinical supervision to volunteers and clinical staff in partner organisations
  6. Promoting the End PJ paralysis initiative on the wards, managing a team of students

 

Service development

  1. Identify and build partnership relationships with an SLA or similar to enable the delivery of the Falls Prevention: Community Exercise Programme in Nursing Homes, GP practices and pre-operative assessment (Kingston Hospital).
  2. Adapt the existing service model to suit new environments of care in the community; ensure SOPs, Delivery Manuals and appropriate documentation are in place to run these services effectively
  3. Recruit and train volunteers to support the delivery of these services in the community on both a 121 (pre-operative assessment / GPs) and class based model (GPs/Nursing Homes)
  4. Establish clear referral pathways to enable patients to be referred into the Falls Prevention: Community Exercise services
  5. Write DPIAs to oversee the information governance and data sharing arrangements with GPs surgeries as necessary for referring into the Falls Prevention: Community Exercise Volunteers programme
  6. Develop an Insight & Impact evaluation model, including 8-week patient follow-up for new services and set up the data collection tools and regular data monitoring to ensure KPIs are met and/or action plans are developed where they are not being met supported by external partner, Helpforce.
  7. Be an active member of the Falls Prevention: Community Exercise Programme Steering Group, providing regular reports back on the progress and impact of the new services

 

Administrative

  1. Issue patient data and client workbooks to the volunteers taking on 121 patients in the community/pre-operatively
  2. Read and monitor volunteer workbooks (pre-operative assessment / GPs) to measure client progress and escalate issues
  3. Add records to Access and CRS (pre-operative assessment patients only)
  4. Agree patient record keeping procedures with partner organisations on a needs-only basis

 

Governance

  1. Report regular performance and impact data to the FP:CE Steering Group as required
  2. Write and end of programme evaluation report for the Kingston Hospital Charity and during the project as requested
  3. Input into the development of further funding bids to secure the sustainability of the programme where data indicates it has been successful.

 

Volunteer Management

  1. Train, support and supervise the activities of Falls Prevention: Community Exercise Volunteers in support of these services
  2. Apply the Volunteering Policy as required, e.g. taking part in the review and performance management of volunteers as required
  3. Process Volunteer Expenses in line with the Volunteering Expenses Policy
