Are you an experienced  Band 6 nurse thinking of a career in Admission Avoidance and Intermediate care service

This is an exciting time to be part of our developing and progressive working environment which is dynamic and proactive with a commitment to making a difference for our patient and careers  

As a senior nurse you will be responsible for providing highly skilled nursing care to patients who reside in the Borough of Tower Hamlets. You will be promoting the health of this population and reducing the effects of ill health, disability and maximising the individuals potential for independence. The primary focus of this role is around prevention and early intervention. This role will play a key part in delivering a community-centred, integrate approach to health and social care, aiming to improve outcome of frail, older adults and reduce the need for hospital admissions.

Our team is part of a wider established integrated service. This will give you the opportunity to work closely with our Physiotherapists, Occupational Therapists, Care Navigators, Mental Health  Nurse,  Physician response team in Royal London, the London Ambulance service and Psychologist.

Our service is also supported by a team of experienced administrators. You will also have the opportunity to work closely with the local General Practices, Tower Hamlets Social Service team and other statutory and non-statutory services to meet the health and social needs of this local population.

Main duties of the job

  • work collaboratively with care homes, reablement services, adult social care, and the frailty pathway to implement a proactive prevention approach.
  • Deliver targeted health education programs to promote self-management and early intervention.

 

  • conduct thorough assessments, identify individuals at high risk, and provide early invention strategies to prevent hospital admissions.
  • Develop individualised care plans that support independence and well being in community settings.
  • Work alongside the B6 Nurse to deliver an MDT approach to care, ensuring comprehensive support for complex cases.
  • Collaborate with voluntary sector partners to enhance health education and promotion, fostering a holistic approach to population health.
  • Stratify caseloads to ensure resources are effectively targeted toward those who would benefit most from preventative care.

Education and Training

Essential criteria
  • • Registered Nurse with evidence of current NMC registration
  • • Specialist Practitioner (District Nursing) Degree or equivalent/ Certificate/diploma
  • Limited prescriber
Desirable criteria
  • • Leadership or management Course
  • • Independent Prescriber (or willingness to undertake)
  • • Physical assessment / First Contact Course (willingness to undertake)
  • specialist practitioner (district nursing) degree or equivalent

Experience

Essential criteria
  • • Experience of working independently, unsupervised providing high quality, evidenced based care.
  • • Able to demonstrate ability to communicate effectively at all levels.
  • • Understanding of the Long Term Conditions management.
  • • Knowledge of Safeguarding Children and Vulnerable Adults policies and procedures
  • • Effective interpersonal, organisational, prioritisation, teamwork and teaching skills.
  • • An ability to develop effective network with stakeholders from Primary and Secondary care.
  • • Able to professionally and personally develop oneself and other team members.
  • • Proactively seeks opportunities for service improvement and solution finding
