Are you looking for an exciting new challenge and a chance to develop your leadership, knowledge and expertise in patient discharge, transfer of care processes and people management?

East London Foundation Trust is looking for an enthusiastic and proactive individual to provide operational leadership to the Transfer of Care Hub Team based in the Royal London Hospital.

The successful individual will operationally manage and lead the team consisting of nursing, therapy, social work, discharge navigator and admin team members. You will be working closely with health and social care partners to ensure safe and timely discharges and minimise barriers, improving patient experience across discharge pathways.

You will be part of a wider senior community services team delivering a discharge to assess model of care in the community.  You will be supported by the Transfer of Care Hub manager and work closely with system partners including acute staff, local authorities, voluntary agencies and the ICB.

Main duties of the job

 

·        A highly competent and skilled leader with experience of operational service management in the NHS.

·        Understand community services business and relationships between providers, commissioners, primary and social care.

·        Ensure effective assessment, planning, implementation and evaluation of discharge pathways meets standards and guidelines.

·        Work with the Performance and Quality Team to ensure key metrics are in place to monitor and report on service performance both internally and externally to key stakeholders.

·        Manage all service complaints within the required time frame and ensure that any learning from the complaint is shared with the team and wider services.

·        Ensure that all care and care planning is evidence-based and meets professional, local and national standards and targets.

·        Work with teams to identify lessons learned from serious untoward incidents, other incidents, complaints, SOVAs and, through any other route, ensures correct process is followed and learning is escalated to the senior team and cascaded to all staff.

·        Ensure that supervision and appraisal is high quality and effective.

·        Take responsibility for out of hours cover and on duty service provision.

·        To liaise with IDH colleagues in other boroughs to support in the discharge of complex cases.

·        Lead on ensuring team compliance with use of digital systems

·        Support with screening referrals

The Transfer of Care Hub (ToCH) is based at The Royal London Hospital and facilitates discharges across the Royal London and Tower Hamlets. It is staffed by East London Foundation Trust (ELFT), London Borough of Tower Hamlets (LBTH) and Barts NHS staff. The purpose of the role job is to operationally manage and lead the Transfer of Care Hub team consisting of nursing, therapy, social work, discharge navigator and admin support team members.

You will provide operational leadership to the Transfer of Care Hub for the Royal London and Tower Hamlets.  You will be responsible for developing and supporting nursing, therapy and LBTH staff in the planning, delivery and support of discharge from acute into community services, ensuring all patients have a safe and timely discharge.

You will be part of a wider senior community services team delivering a discharge to assess model of care delivering high quality nursing, therapy and social care in the community.  You will work closely with system partners including acute staff, local authorities, voluntary agencies and the ICB to ensure that community services are represented appropriately. 

Person specification

Education, Qualification, Traning

Essential criteria
  • • Educated to Masters level in relevant subject or equivalent level qualification or relevant experience of working at a senior level
  • • Evidence of post-qualifying and continuing professional development
Desirable criteria
  • • Registered with the NMC or HCPC
  • • Advanced physical assessment skills

Experience

Essential criteria
  • Experience in operational health service management, managing a teams with AHP / Nursing services
  • Experience managing conflict
  • Experience of planning and delivering programmes, audits and services on time
  • Experience of identifying need, setting up and implementing internal processes and procedures to support efficient service delivery and ability to communicate this at range of levels from across the organisation
  • Significant experience at Senior level of managing highly complex discharges
Desirable criteria
  • Experience Chairing meetings

  • Clear understanding of the background to and aims of current healthcare policy and appreciates the implications of this on engagement and service delivery
  • Proven written and oral communication skills and the ability to articulate information and influence others

  • Highly developed communication skills to allow for delivering key messages to a range of stakeholders/customers both internal and external to the organisation, some at very senior level
