St Thomas’ has one of the largest critical care units in the UK and one of the busiest emergency departments in London. It is also home to Evelina London Children’s Hospital.

Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions. 

Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

In February 2021 the Royal Brompton and Harefield joined Guy’s and St Thomas’ NHS Foundation Trust, bringing together world-leading expertise in the care and research of heart and lung disease. Job overview

Guy’s and St Thomas’s NHS Foundation trust merged with the Royal Brompton and Harefield NHS Foundation trust to form a combined trust covering specialist cardio-respiratory care. As a consequence, the trust includes 2 of the largest home ventilation services in Europe and are seeking a senior clinical fellow to work across sites to gain experience of specialist management of chronic respiratory failure. The post will be split between the Lane Fox service (based at St Thomas’s Hospital and satellite Remeo unit at Redhill) and the Royal Brompton sleep and ventilation service (based at Sydney Street and Fulham road sites).

The Lane Fox Respiratory Service is currently involved in many exciting initiatives, including the establishment in 2014 of the Lane Fox Remeo centre, a 20-bed unit that complements the 14-bed Lane Fox Unit at St Thomas’ Hospital.  The team is keen to appoint an individual with appropriate respiratory and critical care skills to support the in-patient and out-patient management of weaning and chronic respiratory failure. The service is at the forefront of clinical and academic excellence and will expect the appointed individual to work within the existing academic structure to deliver clinically relevant research

Main duties of the job

The Lane Fox Respiratory Service offers a wide range of services to patients with chronic respiratory conditions.  The post-holder will undertake in-patient cover at both the Lane Fox Unit at St Thomas’s Hospital and the Lane Fox Remeo unit supporting patients weaning from invasive ventilation. Out-patient work will include home NIV setup clinic and outreach and community reviews. The successful applicant will be expected to take an active role in education and research within the Lane Fox Respiratory Service.  This is an excellent opportunity to join a first class team in an active and progressive central London teaching hospital environment. 

The Lung failure Care Group at Royal Brompton Hospital manages approximately 6000 patients on CPAP and 2000 on home mechanical ventilation and has an outreach team of 4 AHP, lead by a nurse consultant, who supervise the most dependent patients in their own homes.  Clinical duties within the care group include inpatient care of patients with end stage lung disease, face to face and video review of these patients and review and management of patients undergoing both home sleep studies and more complex sleep investigations including non-respiratory sleep disorder.

Working for our organisation

The Lane Fox Respiratory Service is a national and international referral centre for respiratory weaning, physical rehabilitation and long-term ventilation [non-invasive and invasive].  We support a large population (over 2000 patients) with non-invasive ventilation, and an increasing number of tracheostomy ventilated patients in the community.  We are unique in the UK in providing 24-hour medical, nursing and technical back-up for patients in the community. 

The Lane Fox Unit at St. Thomas’ and The Lane Fox Remeo centre at Redhill, Surrey, is part of the extensive critical care services at GSTT.  This consists of over 120 critical care beds.  Intensive Care is configured as two purpose-built 15-bed units on adjacent floors in the East Wing on the St Thomas' site (EW1 & EW2).   A 10-bed Medical High Dependency Unit in EW10 with a 4-bed Surgical High Dependency Unit on 11 North.  A 13-bed combined ICU/HDU is located at Guy’s.

Qualifications/Education

Essential criteria
  • Eligible for full UK GMC registration and a Licence to Practise
  • On GMC Specialist Register or within six months of attaining CCT in Respiratory & General Internal Medicine.
  • MBBS or equivalent
  • MRCP or equivalent
Desirable criteria
  • BSc or equivalent
  • PhD or equivalent

Clinical experience

Essential criteria
  • ICU experience as a specialist registrar
  • Acute and home mechanical ventilation (non-invasive and invasive ventilation)
  • Respiratory sleep studies
  • Experience in the management of patients with chronic respiratory failure
Desirable criteria
  • Anaesthetic experience including tracheostomy ventilation experience

Clinical Skills

Essential criteria
  • Experience of weaning from mechanical ventilation
