Our Primary Care Offer

What matters to our primary care team at ELFT is delivering care to our patients in partnership with both our patients and the other organisations we work with. By doing this we are able to provide high quality care that is easy to access; the care we would want for the people we love. We are working with our patients, our teams and our partner organisations to transform our services so that they not only do a great job now but also long into the future. To achieve this we work differently, try new things and constantly learn and evolve. 

The primary care services we provide vary from specialist services, providing care to vulnerable and homeless populations, to more mainstream general practice and we work across East London and Bedfordshire. The Long Term Plan sets out a vision for primary care to lead on population health improvement and we work in partnership with mental health, community services, acute services, social care, the voluntary sector and ambulance services to help our local populations stay well.

The role of clinicians and administrators in primary care is evolving, as a part of our primary care team you will use your skills to help contribute to peoples care. Any job in primary care is a challenging job and for our teams to work together our GP’s, nurses, allied health professionals and our administrators all need a system designed to support everyone, so that we can do the best for our patients.

Our primary care directorate team are developing and working on several exciting new concepts to strengthen and provide resilience to general practice. East London Foundation Trust (ELFT) provides a wide range of mental health, community and primary care services across East London, Outer North East London, Bedfordshire and Luton. The trust has an overarching vision to improve the quality of life for all we serve by focusing our efforts on improving population health outcomes, experience of care, staff experience and value. ELFT's vision to improve the quality of life for all those we serve has drawn us into new areas of work, including primary care. As we move forwards to provide more primary care services it is an exciting time to be part of truly transformational change across the whole health and care system. As part of the new primary care directorate within ELFT our team will be involved in leading triple aim population health quality improvement work. Quality of care is our top priority. It is at the forefront of all that we do and is firmly embodied in our mission to provide the highest quality primary, community and mental health care in England by 2020. Sometimes it involves fundamental change in the way things are done, with everyone working together sharing the same passion and commitment.

Prif ddyletswyddau'r swydd

  • Under the direction of the Practice Nurses and ACPs, undertake basic nursing and technical tasks to provide a comprehensive nursing service for Practice patients.
  • Liaise with the Diabetic Team and be responsible for the administration of diabetic care plans.
  • To provide a phlebotomy service for the practice population
  • Undertake routine diagnostic nursing tasks such as, ECGs, blood pressure checks, ambulatory blood pressure monitoring, blood glucose monitoring, routine heights, weights and urinalysis
  • Undertake spirometry (if trained to do so)
  • Use patient contacts as opportunities to promote health education, including education on practice systems such as obtaining a blood result.
  • Participate in health promotion projects such as No Smoking Day
  • Take responsibility for the administration of diabetic care planning, coordinating patient results and clinical observations and ensuring the care plan is sent to the patient in time for their annual review
  • To participate and administer the NHS Health Check Programme
  • Provide vaccination to relevant cohorts (if trained to do so)

The organisation and delivery of primary care services in ELFT is being re-visioned to reflect an aspiration to deliver services in a more integrated manner in collaboration with internal and external partners. To this end the portfolio of primary care services provided directly by the Trust is expanding and the model of service delivery is changing to include more direct participation of service users. Directly provided primary care services range from specialist homeless practices to mainstream general practices and are located across a wide and diverse geography including  London, Bedfordshire and Luton.
